The Questionnaire for individual women included the following modules:<br />

The Questionnaire for Children Under Five 3 was normally administered to mothers of children<br />

under the age of five; however, in cases when the mother was not listed in the household<br />

roster, a primary caretaker for the child was identified and interviewed. The questionnaire<br />

included the following modules:<br />

o Age<br />

o Birth Registration<br />

o Early Childhood Development<br />

o Breastfeeding<br />

o Care of Illness<br />

o Immunization<br />

o Anthropometry<br />

o Blood Test for Anaemia 4<br />

The questionnaires are based on the MICS4 model questionnaire 5 . From the MICS4 model<br />

English version, the questionnaires were translated into Dari and Pashto, and were pre-tested<br />

in Kabul province (Kabul city district and Farza district) and Parwan province (Charikar city<br />

district and Bagram district) during May 2010. Based on the results of the pre-test,<br />

modifications were made to the wording and translation of the questionnaires. A copy of the<br />

<strong>AMICS</strong> questionnaires is provided in Appendix F. In addition to the administration of<br />

questionnaires, fieldwork teams tested the salt used for cooking in the households for iodine<br />

content, observed the place and facilities used for hand washing, measured the weights and<br />

heights of children aged less than five years, and tested the blood of children aged under five<br />

and the blood of women aged between 15 and 49 years. Details and findings of these<br />

measurements are provided in the respective sections of the report.<br />

Training and Fieldwork<br />

Training for the fieldwork was conducted for 21 days in August and September 2010. Training<br />

included lectures on interviewing techniques and the contents of the questionnaires, in addition<br />
