Choices which comb<strong>in</strong>e hospital cover and <strong>in</strong>creased benefits for<br />

primary care cover and are designed specifically for <strong>the</strong> vary<strong>in</strong>g<br />

healthcare needs of people at different life stages,<br />

• Introduction of a primary healthcare package called <strong>Health</strong>Steps<br />

which can be purchased on a standalone basis or as an add-on to<br />

traditional hospital plans,<br />

• Launch of an annual travel <strong>in</strong>surance product <strong>in</strong> February 2004,<br />

• First provider <strong>in</strong> Ireland of dental <strong>in</strong>surance as of May 2004 which is<br />

provided on Vhi <strong>Health</strong>care’s behalf by DeCare,<br />

8.29 On <strong>the</strong> basis of <strong>the</strong>se factors, it is clear that Vhi <strong>Health</strong>care has<br />

responded to new entrant competition by offer<strong>in</strong>g improved products<br />

to its exist<strong>in</strong>g consumers and <strong>in</strong>vest<strong>in</strong>g more <strong>in</strong> attract<strong>in</strong>g new<br />

consumers. Vhi <strong>Health</strong>care does not appear to be act<strong>in</strong>g <strong>in</strong>dependently<br />

of its competitors <strong>in</strong> terms of product and service competition. Product<br />

quality has noticeably improved s<strong>in</strong>ce <strong>the</strong> market was opened to<br />

competition <strong>the</strong>reby <strong>in</strong>dicat<strong>in</strong>g that BUPA Ireland and VIVAS <strong>Health</strong> do<br />

exert competitive pressures on Vhi <strong>Health</strong>care. Vhi <strong>Health</strong>care could<br />

not profitably decrease quality for a susta<strong>in</strong>ed period of time.<br />

Consumer Buyer Power<br />

8.30 This section exam<strong>in</strong>es whe<strong>the</strong>r sufficient buyer power from Vhi<br />

<strong>Health</strong>care’s customers exists to limit Vhi <strong>Health</strong>care’s ability to<br />

exercise market power. Buyers of PHI are made of up of two ma<strong>in</strong><br />

groups – <strong>in</strong>dividuals/families, and corporate buyers who pay for health<br />

<strong>in</strong>surance on behalf of <strong>the</strong>ir employees or members.<br />

8.31 Individual consumers of PHI act<strong>in</strong>g alone are not <strong>in</strong> a position to exert<br />

barga<strong>in</strong><strong>in</strong>g or buyer power vis-à-vis Vhi <strong>Health</strong>care. However, <strong>the</strong><br />

actions of <strong>in</strong>dividuals <strong>in</strong> response to price <strong>in</strong>creases, for example would<br />

be expected to be correlated and may <strong>the</strong>refore act as a competitive<br />

constra<strong>in</strong>t on Vhi <strong>Health</strong>care.<br />

8.32 Large employer-paid group schemes 159 , may be expected to have<br />

buyer power as <strong>the</strong>se are an important source of revenue for Vhi<br />

<strong>Health</strong>care. These schemes are highly profitable given <strong>the</strong> profile of<br />

<strong>the</strong>ir members who tend to be young and healthy. 160 If a large<br />

employer-paid group scheme with a few hundred members was<br />

dissatisfied with <strong>the</strong> service quality and/or price of Vhi <strong>Health</strong>care<br />

products <strong>the</strong>y could <strong>in</strong>vite o<strong>the</strong>r PHI providers to tender for <strong>the</strong><br />

provision of <strong>the</strong>ir health <strong>in</strong>surance requirements.<br />

8.33 However <strong>the</strong> level of barga<strong>in</strong><strong>in</strong>g power of employer-paid group<br />

schemes could be limited for <strong>the</strong> follow<strong>in</strong>g reasons:<br />

• Internal difficulties which could arise with<strong>in</strong> <strong>the</strong> company itself if<br />

management with<strong>in</strong> <strong>the</strong> company is consider<strong>in</strong>g switch<strong>in</strong>g its group<br />
