6.6.5 The form should contain any relevant information about the genetically<br />

manipulated material which falls within these <strong>Guidelines</strong>. In particular, the<br />

following information should be included if appropriate:<br />

• details of the donor organism, which is the DNA source and characteristics<br />

of the genes transferred;<br />

• the method or vector used to transfer the DNA to the host organism;<br />

• host organism;<br />

• complete details should be provided if a plant or animal is infected with a<br />

genetically manipulated micro-organism.<br />

• Risk assessment on import, handling and transport of the genetically<br />

manipulated material.<br />

6.6.6 If appropriate, the relevant agency may make the information provided on this<br />

form available to the GMAC, or to other assessors, and may request specific<br />

information about any aspect of the genetically manipulated material. GMAC’s<br />

assessment of the Application to Import will be accelerated if a small-scale<br />

proposal for the work has already been received.<br />
