6.2 Work- hours and Accident Data<br />

To measure Safety Performance with the goal <strong>of</strong> achieving zero injuries and illnesses, the<br />

recording and classification <strong>of</strong> occupational injuries and illnesses <strong>of</strong> all direct hire workers and<br />

contractors are reported following the industry guidelines in Canada (WCB and CAPP).<br />

For your Direct – Hire Employees and your Subcontractor Employees: In the spaces below,<br />

please record the Total Number <strong>of</strong> Fatalities, Lost Time Cases, Medical Aid Cases and First<br />

Aid Cases and the Total Number <strong>of</strong> Restricted Work Cases, Restricted Medical Aid Cases<br />

and Restricted First Aid Cases. With the exception <strong>of</strong> fatalities, also provide the total number<br />

<strong>of</strong> days away from work for each.<br />

Next, record the number <strong>of</strong> Near Misses, the Total Site Work-hours (Exposure Hours),<br />

Total Number <strong>of</strong> Employees, the Average Full Time Equivalent, and the Number <strong>of</strong><br />

Hours in Your Normal Work Week.<br />

Use WCB and CAPP definitions. If you do not track in accordance with these definitions,<br />

click Unknown in the boxes below.<br />
