Retiree Health Care SPD Effective January 1, 2012 What Happens When You or a Dependent Turn Age 65 or Become Medicare Eligible Before Age 65 If you or a dependent turn age 65 or become Medicare Eligible before age 65 you need to refer to the “MEDICARE ELIGIBLE RETIREES AND DEPENDENTS TURNING AGE 65” section in this SPD and also the “HOW COVERAGE WORKS IF YOU ARE AGE 65 OR OLDER OR PRE-65 AND MEDICARE ELIGIBLE” section in this SPD. 56
Retiree Health Care SPD Effective January 1, 2012 MEDICARE ELIGIBLE RETIREES AND DEPENDENTS TURNING AGE 65 Note: If you are enrolled in Kaiser Colorado, this information does not apply to you. Please see the separate Kaiser materials. What Happens When You Turn Age 65 or Become Medicare Eligible before Age 65 In order to continue coverage under the Program when you turn 65 or become Medicare eligible before age 65, you must enroll yourself and any Medicare eligible dependents in the UnitedHealthcare or Medica Plan option available to you in your area. You will receive information regarding enrollment in the UnitedHealthcare or Medica Plan option approximately 90 days prior to your 65 th birthday. Your enrollment form(s) (for you and any eligible dependents) must be received by the U.S. Bank Employee Service Center and processed, by the deadline on your enrollment materials or you will no longer be enrolled in the Program. The effective date of coverage into the UnitedHealthcare or Medica Plan option will be the first of the month in which you turn age 65 (as long as your UHC/Medica application has been processed). If your birthday is on the first day of the month, then your coverage under the UnitedHealthcare or Medica Plan option will be effective on the first of the prior month (as long as your UHC/Medica application has been processed). If you become Medicare eligible before age 65, you must call the U.S. Bank Employee Service Center at 1-800-806-7009 and request enrollment materials for the UnitedHealthcare or Medica Plan option. If you have non-Medicare eligible dependents under age 65, your dependent will remain in their current Program option. In order to enroll in the UnitedHealthcare or Medica Plan option, you and your Medicare eligible dependents, must be enrolled in Medicare Parts A and B to receive benefits. If you or your dependents do not enroll in Medicare Parts A and B, you or your dependents will not be eligible to continue coverage in the Program. You will receive a new ID card. You will need to show your new ID card, plus your Medicare card, to your health care providers when receiving services. What Happens When a Dependent Turns Age 65 or becomes Medicare Eligible Before Age 65 If you are already enrolled in the UnitedHealthcare or Medica Plan option, your covered dependent must enroll in the UnitedHealthcare or Medica Plan option at the time they turn age 65 or when they become Medicare eligible before age 65. Enrollment information will be provided to your covered dependent 90 days prior to turning age 65. Your dependent must enroll by the deadline on the enrollment materials or your dependent will no longer be enrolled in the Program. The effective date of coverage into the UnitedHealthcare or Medica Plan option will be the first of the month in which your dependent turns age 65 (as long as your UHC/Medica application has been processed).
