<strong>NMICS</strong> <strong>2010</strong>, Mid- and Far Western Regionsgrouped together in nine clusters: one rural cluster each in Achham, Dolpa and Kailali and two ruralclusters each in Baitadi, Bajhang and Humla.Similarly, in cases <strong>of</strong> large wards, especially in urban areas or municipalities, census enumerationblocks were used. Enumeration blocks were created by the GIS Section <strong>of</strong> the CBS for the 2011population census by segmenting large wards. Of the 50 urban clusters in the survey, 22 wereselected from segmented municipalities in the five districts <strong>of</strong> Banke, Dang, Kailali, Kanchanpur andSurkhet. Thus, a total <strong>of</strong> 6,000 households were selected for interviewing, <strong>of</strong> which 1,250represented urban areas (municipalities) and 4,750 represented rural areas (Village DevelopmentCommittees (VDCs)). The sample was stratified by regions and is not self-weighting. Sample weightswere applied in the reporting <strong>of</strong> subregional results. A more detailed description <strong>of</strong> the sampledesign can be found in Appendix A.QuestionnairesThe standard MICS4 questionnaires were used and adapted to include several country-specificquestions and modules. Three sets <strong>of</strong> questionnaires were used in the survey.(i) A household questionnaire used to collect information on all de jure household members (usualresidents), the household, and the dwelling.(ii) A women’s questionnaire administered to all women aged 15–49 years living in each household.(iii) An under-fives’ questionnaire administered to mothers or caretakers <strong>of</strong> all children under five 2living in each household.The Household Questionnaire included the following modules.Household listing formEducationWater and sanitationHousehold characteristicsChild labourDe-worming (Nepal-specific module)Child disciplineHand-washingSalt iodizationThe Questionnaire for Individual Women included the following modules. Woman’s background Access to mass media and use <strong>of</strong> information communication technology Desire for last birth Maternal and newborn health 3 Illness symptoms Contraception Unmet need Attitudes towards domestic violence 4 (Nepal-specific module)2 The terms ‘children under five’, ‘under-fives’, ‘children aged 0–4 years’, and ‘children aged 0–59 months’ are usedinterchangeably in this report.3 Non-MICS standard questions were added to the questionnaire for women aged 15–49 years and asked to mothers whohad given birth in a non-institutional setting in the two years preceding the survey in order to assess whether safe newborncare practices were adopted in the Mid- and Far Western Regions <strong>of</strong> Nepal.4 Non-MICS standard questions were added to the questionnaire for women aged 15–49 years to assess their attitudestowards whether mothers-in-law are justified in verbally abusing or threatening their daughters-in-law.26
