NMICS 2010, Mid- and Far Western RegionsHand-washingHand-washing with water and soap is the most cost-effective health intervention to reduce theincidence of both diarrhoea and acute respiratory infections in children under five. It is mosteffective when done using water and soap and after using a toilet or cleaning a child, before eatingor handling food, and before feeding a child. Monitoring correct hand-washing behaviour at thesecritical times is challenging. When direct observation is not practicable, a reliable alternative to selfreportedbehaviour is assessing the likelihood that correct hand-washing takes place by observing ifa household has a specific place where people most often wash their hands and observing if waterand soap (or other local cleansing materials) are present at that specific place.Table WS.9 shows that 94 percent of households in the MFWR had a specific place for hand-washing.Some five percent of households could not indicate a specific place and less than one percent (0.3percent) did not give permission to see the place used for hand-washing.Of those households where a place for hand-washing was observed, over half (51 percent) had bothwater and soap present at the designated place. In 12 percent of households only water wasavailable and in another 12 percent only soap was available. The remaining 25 percent ofhouseholds had neither water nor soap available at the place designated for hand-washing. Ofhouseholds with an observed hand-washing place, some 48 percent in the Mid-Western Region and55 percent in the Far Western Region had both soap and water available. The highest proportion ofhouseholds with both soap and water available was in the Far Western Terai (73 percent) and thelowest was in the Mid-Western Mountains (27 percent). Urban households (82 percent) were muchmore likely than rural households (47 percent) to have both available. The education of thehousehold head and household wealth status were positive associated availability of soap andwater. Households whose head had at least secondary education (64 percent) were more likely tohave both soap and water available than households whose head had primary education (50percent) or no education (44 percent). Households in the richest quintile (91 percent) were six timesmore likely to have water and soap in than those in the poorest quintile (15 percent).95
