Graduate applications arereviewed on an ongoing or rolling basis.To apply, submit the following:• Bellin College of Nursing Graduate Admission Application• Set of three Graduate Reference/Release Forms• Non-refundable $50 application fee (payable to: Bellin College)• One official copy of all post-high school (e.g. college/university) transcripts. Transcripts must be sentdirectly to Bellin College/Graduate Admissions Department by the college/university to be consideredofficial.• Goal Statement (see goal statement guidelines)• Resume’ or curriculum vita• Three reference forms (professional and/or academic)• Copy of current registered nurse licensure. If license will expire prior to enrollment, an updated licensemust be provided• Test of English as a Foreign Language (TOEFL), if applicablePersonal Interview:A personal interview is required prior to acceptance. When all application materials have been received aninterview will be scheduled. The appointment will be scheduled with the MSN Program Director. Interviewsare scheduled by phone or in person on week days between the hours of 7:30 a.m. and 5:30 p.m. and takeapproximately one hour.Prior to enrollment in the first Practicum course, the student must submit the following:• Criminal Background Information Disclosure Form• Drug Screen• Health RecordsHealth Requirement:Students must provide evidence of meeting the health requirements for the agency in which they completethe practicum in the semester prior to entering the nursing practicum courses.Background Check:Students must have a background check completed in the semester prior to entering the nursing practicumcourses.Goal Statement Guidelines:Please write a two-to-three page, typed document that describes your:• Area of clinical interest• Reasons for desiring a master’s degree in nursing• Career goals• Career plans86
