Partnering for the Public’s <strong>Health</strong><br />

SECTION 4: IMPLEMENTATION AND EVALUATION PLAN<br />

The public health systems priorities are based on the 10 Essential Public<br />

<strong>Health</strong> Services. Members <strong>of</strong> the Executive <strong>Plan</strong>ning Committee identified<br />

the areas <strong>Indiana</strong> Local <strong>Health</strong> Departments can focus on that will impact<br />

the health outcomes <strong>of</strong> their communities the most. Enforcement <strong>of</strong> Laws<br />

and Regulations and Public <strong>Health</strong> Workforce Development were the top<br />

two public health essential services targeted for development for I-<strong>SHIP</strong>.<br />

The remaining five priority areas (Data and Community <strong>Health</strong> Pr<strong>of</strong>iles;<br />

Diagnosing and Investigating <strong>Health</strong> Hazards; Public <strong>Health</strong> Policy<br />

Development; Informing, Educating and Empowering the Public; and<br />

Integrated <strong>Health</strong>care Delivery) will be developed over the next two years<br />

by the Office <strong>of</strong> Public <strong>Health</strong> Performance Management at the <strong>Indiana</strong><br />

<strong>State</strong> Department <strong>of</strong> <strong>Health</strong> (ISDH).<br />

Additional data is to be collected by an Accreditation Assessment Team<br />

(AAT) located at ISDH. The AAT is to conduct a public health<br />

accreditation gap analysis at the local health department level in 2011.<br />

These newly acquired data will be used as both a baseline and evaluation for<br />

the year 1 public health essential services goal development criteria. The<br />

Office <strong>of</strong> Public <strong>Health</strong> Performance Management will utilize the same<br />

methodology to develop the remaining five priority essential services. The<br />

development <strong>of</strong> the remaining services will be based on the final Public<br />

<strong>Health</strong> Accreditation Board standards and measures.<br />

The evaluation <strong>of</strong> the System Priorities will be conducted by the Office <strong>of</strong><br />

Public <strong>Health</strong> Performance Management. The evaluation instrument will be<br />

based on the Local Standards & Measures Self-Assessment Tool developed<br />

by the Public <strong>Health</strong> Accreditation Board. The Local Standards &<br />

Measures Self-Assessment Tool is the foundation for the public health<br />

accreditation gap analysis plan. A series <strong>of</strong> questions are asked to determine<br />

if health departments can demonstrate, partially demonstrate, or not<br />

demonstrate documentation requirements needed to meet each measure for<br />

accreditation. Each Assessment/Standard has an identified Domain in the<br />

public health accreditation documentation; the evaluation will connect the<br />

assessment/standard to the appropriate domain.<br />

To evaluate the local health departments‘ progress toward the identified<br />

system priorities, a survey focusing on each identified Assessment/Standard<br />

will be administered at years 3 and 5 <strong>of</strong> the I-<strong>SHIP</strong>. Data will be reported in<br />

an aggregate format so no individual health department data will be<br />

identifiable.
