Dear <strong>State</strong> <strong>Health</strong> <strong>Improvement</strong> <strong>Plan</strong> Reader:<br />
The <strong>Indiana</strong> <strong>State</strong> Department <strong>of</strong> <strong>Health</strong> is pleased to present the 2011-2016 <strong>Indiana</strong> <strong>State</strong> <strong>Health</strong><br />
<strong>Improvement</strong> <strong>Plan</strong>. The <strong>Indiana</strong> <strong>State</strong> <strong>Health</strong> <strong>Improvement</strong> <strong>Plan</strong> was created among a<br />
collaboration <strong>of</strong> executive committee members who represented a wide variety <strong>of</strong> organizations.<br />
Without their diligent work and commitment this document would not exist.<br />
<strong>Indiana</strong>‘s needed health improvement will only occur with strategic and productive interaction<br />
with all elements that influence Hoosier health. This requires a statewide systematic and<br />
consistent approach that creates a dynamic network <strong>of</strong> health promotion, measured outcomes,<br />
and improved delivery <strong>of</strong> critical services.<br />
All parties interested in public health are encouraged and challenged to review this document and<br />
determine your role in the future <strong>of</strong> moving public health forward in <strong>Indiana</strong>. There are<br />
numerous challenges and opportunities for every entity to play a critical role: hospitals, health<br />
departments, nonpr<strong>of</strong>its, universities, food establishments, surgery centers, etc.<br />
The goals, objectives and activities contained in the <strong>Indiana</strong> <strong>State</strong> <strong>Health</strong> <strong>Improvement</strong> <strong>Plan</strong> are<br />
geared toward improving the lives <strong>of</strong> all <strong>Indiana</strong> residents. This plan will be implemented and<br />
evaluated over the next five years. Successful implementation can only occur through<br />
collaboration among individuals, public, private, governmental, and nonpr<strong>of</strong>it organizations.<br />
The <strong>Indiana</strong> <strong>State</strong> Department <strong>of</strong> <strong>Health</strong> is confident that with strong and committed partners, the<br />
<strong>Indiana</strong> <strong>State</strong> <strong>Health</strong> <strong>Improvement</strong> <strong>Plan</strong> will move forward in a successful manner.<br />
