Q: Was the intention to replace the original hand-written report with a typed<br />

one?<br />

A: Dr Shah and Dr Ishaque did not sign the report. Dr Tariq Mirza had told<br />

me that Dr Shah had told him that he would like to talk before signing it and<br />

then I contacted Dr Shah.<br />

Dr Shah had called us for a meeting on Oct 5 with Tariq Mirza. We reached<br />

his house at 4:15 pm and we remained there until 6:30 pm, and during more<br />

than two hours, I offered my Asr and Maghreb prayers. We had to wait for<br />

nearly 30 minutes to see Dr Shah. We showed him both the reports to which<br />

he said they are two different reports. Then Dr Shah suggested a meeting<br />

with the doctor who had operated upon ASP Shahid Hayat at AKUH.<br />

Q: From the house of Dr Shah, how many time you contacted the health<br />

secretary over phone?<br />

A: Only one call was made and I also gave the phone to Dr Shah to talk to the<br />

health secretary.<br />

Q: In Dr Mirza's evidence it has come on record that more than 2-3 calls were<br />

made and one call was received from the health secretary.<br />

A: Today's statement of mine is correct. I told the health secretary over phone<br />

that there would be some delay in submitting the report because Dr Shah had<br />

suggested to examine the doctor at AKUH for a first-hand account from him.<br />

Then we two returned from his house. Then we assembled at AKUH on Oct<br />

7, except for one doctor all were present and Dr Shahzad, of AKUH, was not<br />

available to us because he was busy. Then I showed both reports to Dr<br />

Mushtaq Ahmed, to which he said there can't be two reports. We agreed to<br />

submit the original hand-written report.<br />

The chairman observed that the way the doctors had handled the situation<br />

they have destroyed the credibility of their organisation and there is no<br />

discipline in their conduct and no yard stick for their judgments.<br />

Q: The hand-written report of Sept 30 was kept secret until Oct 7. If the<br />

original hand-written report contained "mistakes" why did you allow the<br />

same after seven days to be sent to the health secretary, which meant that you<br />

are ready to submit any report and amend any report as long as it suits you.<br />
