Electronic Rate Qualifications<br />

To expedite the certification process, you may fax your completed forms to<br />

Upon receiving the properly completed PS Form 1357-S and PS Form 5051:<br />

1. The Postal Service will assign a Logon ID<br />

and password.<br />

2. The Postal Service will send you a test kit<br />

either via e-mail, when an e-mail address is<br />

provided on the application, or by mail.<br />

3. You will submit an electronic test file when<br />

you receive your test kit. Testing your<br />

electronic file for certification includes the<br />

following:<br />

File transmission.<br />

Format, length, and content of the files.<br />

If you are printing your<br />

own labels, at least 20<br />

samples must match the<br />

electronic file. If you plan<br />

to use multiple services,<br />

you must include at least<br />

10 labels per service to<br />

match the file.<br />

Accuracy of linking the unique Package Identification Code (PIC)<br />

to your test barcode labels and to the appropriate record in the<br />

file (for either preprinted labels or labels you print yourself).<br />

4. The National Customer Support Center (NCSC) will test the quality of<br />

the file content and will review any errors and warnings with you.<br />

5. You will receive PS Form 3152, <strong>Confirmation</strong> <strong>Services</strong> Certification,<br />

when the electronic file is certified. The NCSC will sign and date the<br />

Barcoded Labels certification block of the form. A sample of a<br />

completed PS Form 3152 is shown in Appendix H.<br />

Mailers who use the electronic option, print their own labels, presort, or<br />

use permit imprint mailings must submit a copy of the original PS Form<br />

3152, provided by the Postal Service, with their first mailing using a<br />

confirmation service. However, mailers who wish to obtain an electronic<br />

entry scan event (reflected in the extract file as an 03 Accept or Pickup<br />

event for the electronic file number) should submit a PS Form 3152 with<br />

the electronic file number in either a barcode format (preferred) or<br />

written on the form. The mailer should keep the original signed<br />

certification form on file, in case the Postal Service requests it later. The<br />

mailer must present the form to the Postal Service within 24 hours of<br />

request.<br />

Single-piece, postage-evident mailings using the electronic option rate<br />

do not require submission of PS Form 3152. These include mailings<br />

generated from the Postal Service online shipping label application at<br />

www.usps.com or labels generated from a certified third-party vendor or<br />

system that supports electronic option rates. All labels must meet the<br />

format and design guidelines stated in the DMM and in <strong>Publication</strong> <strong>91</strong>,<br />

including the “e” (electronic rate) endorsement in the barcode text line;<br />

example: e/ USPS Delivery <strong>Confirmation</strong>.<br />
