Symbology:
2 digits — Reserved for Application Identifier
“91” in UCC/EAN Code 1282 digits. Must be ‘50’
which is the Service Type Code for an electronic
file.
9 digits — D-U-N-S Number.
8 digits — Variable sequential number up to 8
digits.
1 digit — MOD 10 check digit.
2 Shaded file types 1, 4, and 5 are for future
applications.

PS Form 3152, Confirmation Services Certification.
Package Identification Code (PIC) and sequential
number must be unique for 12 months.
Fixed-length sequential number would be right
justified with a total of 8 digits with leading zeros.
