A small proportion of diseases are caused by a s<strong>in</strong>gle agent that is both necessary<strong>and</strong> sufficient <strong>in</strong> itself to cause a clearly def<strong>in</strong>ed disease. A set of rules (Koch’sPostulates) often can be used to establish a cause–effect relationship <strong>in</strong> this type ofdisease. However, <strong>the</strong> great majority of diseases, <strong>in</strong> all species, are of more complexcausation. Agents may cause disease under certa<strong>in</strong> conditions but not under o<strong>the</strong>rs,multiple agents may be required to produce disease, or several agents may <strong>in</strong>dependentlycause similar disease. In many diseases <strong>the</strong>re is a complex web of causation that may<strong>in</strong>volve many <strong>in</strong>ter-related factors. <strong>Disease</strong> must be considered <strong>in</strong> <strong>the</strong> context <strong>in</strong> which itoccurs. Koch’s Postulates are generally <strong>in</strong>adequate <strong>for</strong> establish<strong>in</strong>g cause-effectrelationships, <strong>and</strong> o<strong>the</strong>r criteria that <strong>in</strong>clude epidemiological <strong>in</strong><strong>for</strong>mation are moreappropriate.Investigation of disease <strong>in</strong>volves answer<strong>in</strong>g five basic questions: Who? Where?When? What? <strong>and</strong> Why? It is impossible to predict <strong>in</strong> advance which discipl<strong>in</strong>es ordiagnostic techniques will be required to solve a problem <strong>and</strong> no one discipl<strong>in</strong>e isomnipotent. However, field <strong>and</strong> laboratory studies must be related back to <strong>the</strong> casedef<strong>in</strong>ition (i.e., Are we look<strong>in</strong>g at <strong>the</strong> same disease?) <strong>and</strong> <strong>the</strong> results should be used toref<strong>in</strong>e <strong>the</strong> def<strong>in</strong>ition of <strong>the</strong> touchstone.243
