Only disidentified data is used in discussions with third party organisations and the confidentiality of the reporter<br />

is assured in any contact with an external organisation. No personal details are retained from reports received.<br />

After ensuring that the report contains all relevant information, all personal details are deleted when the report is<br />

closed with an acknowledgement letter sent to the reporter. Each report is allocated unique reference identification.<br />

After the deletion of personal details, CHIRP is unable subsequently to contact the reporter. The reporter may, if<br />

he/she wishes, contact the CHIRP office for additional information by using the report reference identification.<br />

16.6.3 CHIRP FEEDBACK: The newsletter Maritime FEEDBACK is published quarterly by CHIRP Maritime, covering<br />

current topics from disidentified reports, and including new report forms, and is distributed widely, currently with<br />

a hard copy circulation of around 38,000. Some reports are disidentified further (e.g. time and place, company,<br />

reporter's gender) before being published in FEEDBACK. FEEDBACK maintains an awareness of Human Factors<br />

issues within the broad maritime sector, worldwide, and provides a forum for discussion. CHIRP uses Facebook<br />

to comment on current maritime safety issues, particularly those in which confidential reporting of hazardous<br />

incidents could contribute to improvement.<br />

16.6.4 Confidential Reporting through CHIRP Maritime: Operators and other persons involved with the design,<br />

construction and operations of MASS are encouraged to engage with and learn from the CHIRP Maritime<br />

programme. Online copy of the CHIRP Maritime FEEDBACK and more information about the programme is<br />

available at the CHIRP Maritime programme website: https://www.chirpmaritime.org/<br />

Confidential reports should be submitted to CHIRP Maritime by use of the on-line reporting process, at the<br />

following link: https://report.chirpmaritime.org/submit.aspx<br />

The process of reporting and all guidance necessary is provided as the submission is made. But all persons<br />

making a report should be aware that all information provided is treated by CHIRP Maritime and its contributing<br />

professionals with the utmost confidentiality.<br />
