The HSE has adopted a Population <strong>Health</strong> model <strong>of</strong> health and social care, which takes a<br />

more proactive approach on maximis<strong>in</strong>g <strong>the</strong> health and well-be<strong>in</strong>g <strong>of</strong> <strong>the</strong> population. A key<br />

part <strong>of</strong> this is <strong>the</strong> development <strong>of</strong> Primary Care Teams, and this development is a primary<br />

strategic aim with<strong>in</strong> PCCC. Primary Care Teams will serve resident populations <strong>of</strong> between<br />

4,000 to 10,000, and <strong>the</strong>ir composition will <strong>in</strong>clude General Practitioners, Public <strong>Health</strong><br />

Nurses, Community Therapists, Social Workers and Home Care Support Workers. These<br />

teams will be supported by a Primary and Social Care Network serv<strong>in</strong>g catchments <strong>of</strong><br />

between 30,000 to 50,000, where a broader range <strong>of</strong> <strong>services</strong> will be available.<br />

In <strong>the</strong> community, <strong>the</strong>re is also private nurs<strong>in</strong>g home provision and across <strong>the</strong> 40 homes,<br />

<strong>the</strong>re is a total complement <strong>of</strong> 1,747 beds as presented <strong>in</strong> Table 25 below.<br />
