An action plan for <strong>acute</strong> and community <strong>services</strong><br />

In summary, <strong>the</strong>se national transformation priorities are sett<strong>in</strong>g a direction <strong>of</strong> travel <strong>in</strong> which<br />

more care is provided at home or as close to home as possible, and that when patients do<br />

need <strong>acute</strong> <strong>hospital</strong> <strong>services</strong>, <strong>the</strong>y will receive high quality care that is <strong>in</strong>tegrated with local<br />

<strong>services</strong> and provides best value.<br />

<strong>Review</strong> <strong>of</strong> <strong>acute</strong> <strong>hospital</strong> <strong>services</strong> <strong>in</strong> <strong>the</strong> <strong>Mid</strong>-West<br />

The HSE commissioned Horwath Consult<strong>in</strong>g Ireland Ltd and Teamwork Management<br />

<strong>Service</strong>s Ltd to undertake a review <strong>of</strong> <strong>acute</strong> <strong>hospital</strong> <strong>services</strong> <strong>in</strong> <strong>the</strong> HSE <strong>Mid</strong>-West, us<strong>in</strong>g<br />

recognised <strong>in</strong>ternational best practice as <strong>the</strong> comparator. We were asked to conduct our<br />

study on an <strong>in</strong>dependent ‘fact-f<strong>in</strong>d<strong>in</strong>g’ basis, as opposed to undertak<strong>in</strong>g a process <strong>of</strong> ‘formal’<br />

consultation with all <strong>in</strong>terested parties.<br />

Our primary objectives <strong>of</strong> this review, <strong>the</strong>refore, are to:<br />

• Assess <strong>the</strong> appropriateness or o<strong>the</strong>rwise <strong>of</strong> current <strong>acute</strong> <strong>hospital</strong> <strong>services</strong> <strong>in</strong> <strong>the</strong> <strong>Mid</strong>-<br />

West;<br />

• Def<strong>in</strong>e what <strong>the</strong>se <strong>services</strong> need to look like <strong>in</strong> <strong>the</strong> future if <strong>the</strong>y are to be <strong>in</strong> a position to<br />

deliver <strong>services</strong> <strong>in</strong> l<strong>in</strong>e with <strong>in</strong>ternational best practice standards;<br />

• Determ<strong>in</strong>e <strong>the</strong> optimal support<strong>in</strong>g governance structure for <strong>the</strong>se <strong>acute</strong> <strong>services</strong>; and<br />

• Draw up an action plan to move safely from today’s model <strong>of</strong> care to <strong>the</strong> best practice<br />

model <strong>of</strong> care.<br />

<strong>Review</strong> <strong>of</strong> PCCC and its <strong>in</strong>terface with <strong>acute</strong> care<br />

To ensure full <strong>in</strong>tegration <strong>of</strong> <strong>the</strong> <strong>acute</strong> and <strong>the</strong> community aspects <strong>of</strong> <strong>the</strong> HSE Transformation<br />

Programme, <strong>the</strong> orig<strong>in</strong>al brief was extended to exam<strong>in</strong>e those elements <strong>of</strong> <strong>the</strong> PCCC<br />

<strong>services</strong> that directly relate to <strong>the</strong> <strong>acute</strong> <strong>hospital</strong> sector. We are now also required to report<br />

on:<br />

• Estimat<strong>in</strong>g <strong>the</strong> future resources that PCCC will need if <strong>the</strong>y are to reach <strong>the</strong>ir full potential<br />

for help<strong>in</strong>g patients avoid <strong>acute</strong> admission to <strong>hospital</strong> or be safely discharged from<br />

<strong>hospital</strong> at an earlier stage <strong>in</strong> <strong>the</strong>ir recovery; and<br />

• Identify<strong>in</strong>g <strong>the</strong> PCCC actions required to achieve <strong>the</strong>ir full potential, and <strong>in</strong>tegrat<strong>in</strong>g <strong>the</strong>se<br />

actions <strong>in</strong>to <strong>the</strong> overall action plan for deliver<strong>in</strong>g <strong>the</strong> new strategy for <strong>acute</strong> <strong>services</strong> <strong>in</strong> <strong>the</strong><br />

<strong>Mid</strong>-West.<br />
