An action plan for <strong>acute</strong> and community <strong>services</strong><br />
• Undertak<strong>in</strong>g an estates review to assess compliance <strong>of</strong> <strong>the</strong> current programme with <strong>the</strong><br />
new model <strong>of</strong> <strong>acute</strong> and community care and manag<strong>in</strong>g any future capital <strong>in</strong>vestment<br />
and dis<strong>in</strong>vestment <strong>in</strong> <strong>the</strong> <strong>Mid</strong>-West; and<br />
• Ensur<strong>in</strong>g resources are identified and made available to support <strong>the</strong> transitional<br />
arrangements and any capital and revenue implications <strong>of</strong> <strong>the</strong> <strong>Mid</strong>-West transformation<br />
programme.<br />
Us<strong>in</strong>g best practice standards for cl<strong>in</strong>ical change management<br />
Most cl<strong>in</strong>ical and organisational change occurs <strong>in</strong>crementally and slowly. Most health<br />
pr<strong>of</strong>essionals have experience at this level, very few have any exposure to, or confidence <strong>in</strong>,<br />
a whole system change.<br />
This implementation programme is a whole system change. We have developed a unique<br />
understand<strong>in</strong>g and expertise <strong>in</strong> mentor<strong>in</strong>g health planners, cl<strong>in</strong>icians, <strong>acute</strong> service and<br />
community organisations through whole system change, us<strong>in</strong>g best practice standards as <strong>the</strong><br />
basis for that change, from <strong>in</strong>itial understand<strong>in</strong>g, to cl<strong>in</strong>ical engagement, skills transfer, action<br />
plann<strong>in</strong>g and on <strong>in</strong>to implementation.<br />
The implementation plan for <strong>the</strong> <strong>Mid</strong>-West needs to be drawn up with <strong>the</strong>se characteristics <strong>in</strong><br />
m<strong>in</strong>d.<br />
