<strong>Review</strong> <strong>of</strong> <strong>acute</strong> <strong>hospital</strong> <strong>services</strong> <strong>in</strong> HSE <strong>Mid</strong>-West<br />

An action plan for <strong>acute</strong> and community <strong>services</strong><br />

There is a clear <strong>in</strong>ternational trend towards <strong>the</strong> centralisation <strong>of</strong> specialist <strong>acute</strong> <strong>services</strong> to<br />

ensure <strong>the</strong>re is a sufficient concentration <strong>of</strong> a ‘critical mass’ <strong>of</strong> patient volumes to optimise<br />

<strong>the</strong> provision <strong>of</strong> safe, effective and high quality <strong>services</strong> through a well resourced and highly<br />

tra<strong>in</strong>ed workforce 92 .<br />

Typically, <strong>acute</strong> regional ‘centres <strong>of</strong> excellence’ will provide co-located 24/7 <strong>services</strong> for:<br />

• Major accidents and emergency;<br />

• Trauma;<br />

• Emergency medic<strong>in</strong>e, <strong>in</strong>clud<strong>in</strong>g cardiology, gastroenterology, respiratory medic<strong>in</strong>e,<br />

endocr<strong>in</strong>ology, rheumatology;<br />

• Emergency surgery, <strong>in</strong>clud<strong>in</strong>g general, trauma, urological, vascular, ENT and<br />

ophthalmology;<br />

• Complex planned surgery, applicable to all <strong>acute</strong> surgical specialties;<br />

• Critical care;<br />

• Cancer <strong>services</strong>;<br />

• Obstetrics;<br />

• Acute psychiatry;<br />

• Paediatrics; and<br />

• Neonatology.<br />

These <strong>acute</strong> <strong>services</strong> will have a full range <strong>of</strong> 24/7 cl<strong>in</strong>ical support <strong>services</strong>, <strong>in</strong> particular,<br />

‘high tech’ diagnostics and pathology <strong>services</strong>.<br />
