An action plan for <strong>acute</strong> and community <strong>services</strong><br />
Us<strong>in</strong>g ‘real time’ telemetry, Cathal’s condition, his pulse, blood pressure, ECG and pulse oximetry, are<br />
already be<strong>in</strong>g monitored by <strong>the</strong> duty team <strong>in</strong> <strong>the</strong> CCU. His condition is regarded as stable, but he is<br />
still hypotensive. The ECG confirms an <strong>in</strong>farct. The AP starts thrombolysis after confirm<strong>in</strong>g that Cathal<br />
meets <strong>the</strong> criteria for treatment.<br />
Fours hours later, Cathal is <strong>in</strong> <strong>the</strong> cardiac ca<strong>the</strong>ter laboratory. The duty cardiologist <strong>in</strong>serts a coronary<br />
artery stent across <strong>the</strong> blockage to restore <strong>the</strong> circulation. This results <strong>in</strong> immediate improvement <strong>in</strong><br />
Cathal’s cardiac status and he is admitted to <strong>the</strong> CCU for close monitor<strong>in</strong>g.<br />
Three days later, Cathal is discharged <strong>in</strong>to <strong>the</strong> care <strong>of</strong> <strong>the</strong> cardiac rehabilitation service, an outreach<br />
programme delivered at his local centre. He may not be able to f<strong>in</strong>d <strong>the</strong> solution to his bus<strong>in</strong>ess<br />
worries, but he knows his heart is be<strong>in</strong>g well looked after. Time to learn from <strong>the</strong> experience.<br />
In <strong>the</strong> next section we look at develop<strong>in</strong>g <strong>the</strong> implementation plan to deliver <strong>the</strong> new model <strong>of</strong><br />
care with<strong>in</strong> <strong>the</strong> HSE <strong>Mid</strong>-West.<br />
