Organisational <strong>in</strong>frastructure:<br />
<strong>Review</strong> <strong>of</strong> <strong>acute</strong> <strong>hospital</strong> <strong>services</strong> <strong>in</strong> HSE <strong>Mid</strong>-West<br />
An action plan for <strong>acute</strong> and community <strong>services</strong><br />
• The regional critical care centre is at <strong>the</strong> heart <strong>of</strong> <strong>the</strong> regional ‘centre <strong>of</strong> excellence’;<br />
• The service is led by a consultant team <strong>of</strong> at least 8, provid<strong>in</strong>g 24/7 supervision and <strong>in</strong>put<br />
across <strong>the</strong> cl<strong>in</strong>ical network, with some consultants work<strong>in</strong>g on a full-time basis for critical<br />
care only, and o<strong>the</strong>rs also support<strong>in</strong>g <strong>the</strong> regional anaes<strong>the</strong>tic and pa<strong>in</strong> management<br />
service;<br />
• There is a dedicated operational management structure to ensure <strong>the</strong> smooth day-to-day<br />
runn<strong>in</strong>g and organisation <strong>of</strong> <strong>the</strong> cl<strong>in</strong>ical network;<br />
• There are dedicated robust triage, transfer and transport arrangements <strong>in</strong> place for both<br />
admission and transfer home <strong>of</strong> patients; and<br />
• The <strong>in</strong>tensivist team lead a system <strong>of</strong> critical care education and tra<strong>in</strong><strong>in</strong>g for <strong>the</strong> whole<br />
network.<br />
Planned care network<br />
The local elements <strong>of</strong> <strong>the</strong> network:<br />
• The local ‘centres <strong>of</strong> excellence’ will provide a wide range <strong>of</strong> <strong>services</strong>, <strong>in</strong>clud<strong>in</strong>g<br />
outpatient, day surgery and diagnostic test<strong>in</strong>g. The benefits <strong>of</strong> <strong>the</strong>se centres are <strong>the</strong><br />
provision <strong>of</strong> local access, dedicated facilities, and <strong>the</strong> development <strong>of</strong> new models <strong>of</strong> care<br />
enhanc<strong>in</strong>g <strong>the</strong> quality <strong>of</strong> patient care.<br />
At <strong>the</strong> regional centre:<br />
• All major planned care is undertaken <strong>in</strong> <strong>the</strong> regional centre; and<br />
• There is separate stream<strong>in</strong>g and bed provision for planned <strong>in</strong>-patient and for emergency<br />
admissions to ensure no <strong>in</strong>terruptions <strong>of</strong> planned procedure activities.<br />
Cancer care network<br />
The local elements <strong>of</strong> <strong>the</strong> network:<br />
• Day case and outpatient medical oncology <strong>services</strong> to be provided by <strong>the</strong> local ‘centre <strong>of</strong><br />
excellence’;<br />
• Appropriate care closer to home or at home, with access to supportive and rehabilitative<br />
<strong>services</strong> <strong>in</strong>clud<strong>in</strong>g pa<strong>in</strong> management, psychosocial support and end <strong>of</strong> life care; and<br />
• Integrated, multidiscipl<strong>in</strong>ary team work<strong>in</strong>g <strong>in</strong>volv<strong>in</strong>g primary, community and social care<br />
<strong>services</strong>.<br />
