Whilst it is widely understood that compar<strong>in</strong>g cl<strong>in</strong>ical resource utilisation is not without its<br />

flaws and weaknesses, not least <strong>of</strong> which is ensur<strong>in</strong>g that comparisons are be<strong>in</strong>g made<br />

across similarly provided <strong>services</strong> (hence <strong>the</strong> difficulty with national and <strong>in</strong>ternational<br />

comparators), this section has provided an <strong>in</strong>itial assessment <strong>of</strong> current service provision and<br />

shown that <strong>the</strong>re are significant differences <strong>in</strong> <strong>the</strong> availability and utilisation <strong>of</strong> resources<br />

across HSE <strong>Mid</strong>-West, which are worth fur<strong>the</strong>r, more detailed analysis. Most notable <strong>of</strong> <strong>the</strong>se<br />

differences, are <strong>hospital</strong>isation rates, bed occupancy, and <strong>the</strong> distributed nature <strong>of</strong> <strong>the</strong><br />

consultant medical workforce.<br />

The <strong>hospital</strong>isation rates show that <strong>the</strong> closer a resident lives to a large <strong>acute</strong> <strong>hospital</strong>, <strong>the</strong><br />

more use that is made <strong>of</strong> that facility. In Limerick City, <strong>the</strong> <strong>hospital</strong>isation rate for non-elective<br />

medical admissions is more than twice rate for Limerick County. This is referred to as <strong>the</strong><br />

‘distance decay’ relationship between residents and <strong>hospital</strong> location.<br />

Bed occupancy levels vary across sites. There would appear to be a number <strong>of</strong> <strong>acute</strong><br />

<strong>hospital</strong>s which are experienc<strong>in</strong>g least bed pressures, particularly <strong>in</strong> <strong>the</strong> St. John’s Hospital.<br />

The pr<strong>of</strong>ile <strong>of</strong> <strong>the</strong> consultant medical workforce <strong>in</strong>dicates that <strong>the</strong>re are as many split site<br />

posts, as <strong>the</strong>re are posts based on a s<strong>in</strong>gle <strong>hospital</strong> site <strong>in</strong> <strong>the</strong> <strong>Mid</strong>-West.<br />

Current governance arrangements for <strong>hospital</strong>s<br />

Under present circumstances, <strong>the</strong> management <strong>of</strong> <strong>the</strong> five <strong>acute</strong> <strong>hospital</strong> facilities owned by<br />

<strong>the</strong> HSE (<strong>the</strong> MWRH sites at Dooradoyle, Ennis and Nenagh, and <strong>the</strong> orthopaedic and<br />

obstetric <strong>hospital</strong>s) comes under <strong>the</strong> jurisdiction <strong>of</strong> <strong>the</strong> National Hospitals Office (<strong>Mid</strong>-West).<br />

A General Manager, based at Dooradoyle, has lead responsibility for operational service<br />

delivery, and is supported by locally-based managers/adm<strong>in</strong>istrators <strong>in</strong> each <strong>of</strong> <strong>the</strong> o<strong>the</strong>r<br />

locations.<br />
