Primary Care Teams and wider network support, or a stay <strong>in</strong> a rehabilitation/community<br />
support bed; and<br />
• On average, <strong>the</strong> length <strong>of</strong> stay for rehabilitation/community support would be up to four<br />
weeks, given <strong>the</strong> range <strong>in</strong> <strong>in</strong>tensity <strong>of</strong> care required.<br />
