What steps should be taken to ensure smooth outpatient-to-inpatient and inpatient-tooutpatient transitions? Considerations • DOSING WEIGHT! • Patient may not “know” dose but MUST confirm – Knows number of vials needed to prepare cassettes – Dilutions – Often “mL per day” – Timing of next cassette/bag • Pump availability 32
