AGENDA COVER MEMORANDUM Page 2 C.M. #________ Date:__________ The Clinic will offer pre-employment physicals, primary care to eligible employees and their dependents, prescription dispensing, disease management, injections, exams and screening, as well as, occupational health services including, but not limited to, evaluation and treatment of work related injuries and illnesses and occupational health exams. In an effort to attract employees to the Clinic, employees who use the Clinic will not incur out of pocket expenses. The Agreement provides for a term of three (3) years with an option to renew for two (2) consecutive one (1) year terms. The Clinic will operate for a minimum of 40 hours per week with the initial hours of operation being Monday and Friday 7am - 4pm; Tuesday and Thursday 10am - 7pm; Wednesday 7am - 12 noon and Saturdays 8am to 1pm. These hours will be evaluated and modified as necessary to meet the needs of the City. The Clinic will be staffed by a Physician, Nurse Practitioner or Physician’s Assistant, Nurse and a Medical Technician. The annual management fee to First Onsite will be the lesser of $131,040 or 20% of the salary wages and benefits for the personnel. Furthermore, the Agreement provides for Performance Guarantees, whereby a portion of the management fee is at risk of forfeiture for under-performance of specified benchmarks. A Review Board; consisting of outside healthcare professionals and staff, will be established to provide oversight of the clinic. The Agreement authorizes the Mayor to execute amendments to the Agreement for operational changes such as hours of operation. The proposed location for the Clinic is 464 Fern Street, a vacant historic building which is owned by the City and centrally located to City operations. The windows are badly deteriorated and will be replaced with hurricane resistant windows. Interior buildout will include electrical, plumbing and structural repairs to facilitate three (3) examination rooms, an imaging/x-ray room, a small lab, medical storage area, patient waiting area and administrative offices. The Fern street location provides for adequate patron parking consisting of four (4) on-site parking in the alley and two dedicated on-street parking spaces. The building is also equipped with an existing handicap ramp. To maintain the historic integrity of the structure, there will be no changes to the building exterior and the interior buildout will not jeopardize the historic character. The budget needed for Fiscal Year 2011 is: $200,000 Building Renovations 90,000 Equipment (x-ray machine) 85,000 Start-up Expenses (includes hearing and eye testing equipment for firefighter physicals) 225,000 July - September contract with H2U Wellness for staff, supplies, and drugs $600,000 Total In an effort to control cost, the required landscaping of the property and parking lot repairs and striping will be done in-house by our Parks Maintenance and Public Works employees.
