Apêndice Selection of studies and data extraction Authors will screen the title and abstracts of publications obtained by the search strategy. Unpublished data will be requested from the primary authors when necessary. A standard form will be used to extract the following data: characteristics of the study (design, methods of randomisation, withdrawals / dropouts); participants (age, gender); intervention (type of NPPV, timing and duration of therapy, co-interventions); control (agent and dose); outcomes (types of outcome measures, timing of outcomes, adverse events); and results. This data will then be entered into Review Manager 4.2.7 (RevMan 2003) by a third party who will also identify and resolve discrepancies in the data extraction forms. Study quality grading Methodological quality of the included trials will be assessed with particular emphasis on allocation concealment, intention-to-treat analysis and loss to followup (Schulz 1995). We will also consider obtaining informed consent from participants as a quality criteria. 1. Allocation Concealment: A. Adequate - Randomisation method described such that it would not allow investigator/participant to know or influence intervention group before eligible participant entered in the study. B. Unclear - Randomisation stated but no information on method used available. C. Inadequate - Method of randomisation used such as alternate medical record numbers or unsealed envelopes; any information in the study that indicated that investigators or participants could influence intervention group. D. Not done. 2. Intention-to-treat analysis: Yes - Specifically reported by authors that intention-to-treat analysis was undertaken and this to be confirmed on study assessment. No - Not reported and lack of intention-to-treat analysis confirmed on study assessment. (Patients who were randomised were not included in the analysis because they did not receive the study intervention, they withdrew from the study or were not included because of protocol violation). Not stated. 3. Completeness to follow-up: Percentage of participants excluded/lost to follow-up. Statistical analysis Data from all trials will be analysed using RevMan 4.2.7(RevMan 2003). We will pool continuous variables (such as measurement of blood gases, vital signs) using the weighted mean difference (WMD) or standardized mean difference (SMD - using a scoring system) with 95% confidence intervals (95%CI).
Apêndice We will pool dichotomous variables using relative risk (RR) with 95%CIs. The number needed to treat (NNT) will be calculated for statistically significant results. Data will be pooled using the random effects model. A fixed effects model will also be used to ensure robustness of the model chosen.
