Thoracolumbar braces are widely used<br />

in the care of patients with vertebral fractures<br />

due to osteoporosis.Their usefulness,<br />

however, has never been tested under standardized<br />

conditions.To our knowledge, this<br />

is first randomized, controlled, <strong>pro</strong>spective,<br />

cross over study to determine the efficacy<br />

of an orthosis in the treatment of spinal<br />

osteoporosis.<br />

Spinomed im<strong>pro</strong>ved posture, trunk<br />

muscle strength, lung function, body sway,<br />

and quality of life in postmenopausal<br />

women with osteoporosis and may therefore<br />

play an integral part in the rehabilitation<br />

<strong>pro</strong>cess of osteoporotic patients. The<br />

efficacy of thoracolumbar braces needs to<br />

the further investigated in <strong>pro</strong>spective, randomized<br />

and controlled clinical trials.<br />

CBFA 1 EXPRESSION AND ITS<br />

RELATIONSHIP TO THREE DIMEN-<br />

SIONAL TISSUE STRUCTURE IN<br />

HUMAN CHONDROCYTES IN VITRO<br />

CBFA 1 is a gene enconding a transcription<br />

factor of the runt superfamily and one<br />

of the earliest markers of osteoblastic differentiation.<br />

Recent studies have shown as<br />

well its importance for the maturation of<br />

chondrocytes as well as for enchondral ossification<br />

in CBFA 1 deficient mice. Obviously,<br />

there is an additional role of CBFA 1 in<br />

the transdifferentiation of chondrocytes<br />

into bone cells. However, regulation of<br />

CBFA 1 gene expression is not yet understood.We<br />

tested the hypothesis,if CBFA 1 is<br />

present in human articular chondrocytes<br />

and if a regulation of its expression can be<br />

achieved from two to three dimensional in<br />

vitro conditions.<br />

The imunohistochemical stainings<br />

revealed differentited chondrocytes with<br />

a densely stained expression of collagen<br />

type II. In all human articular chondrocytes<br />

examined a CBFA 1 expression could be<br />

demonstrated. Under monolayer in vitro<br />

conditions, an increasing CBFA 1 mRNA<br />

expression was detectable during subsequent<br />

passages. A significant increase up to<br />

200 % of passage 1 could be observed.<br />

Using the alginate based bead system and<br />

thus <strong>pro</strong>viding a three dimensional tissue<br />

structure in vitro, the CBFA 1 level was only<br />

maintained or even decreased.<br />

This study demonstrates a regulation of<br />

CBFA 1 expression dependent on a two or<br />

three dimensional tissue structure in<br />

human chondrocytes in vitro.This observations<br />

suggest that for establishing and maitenance<br />

of a stable cell differentation the tissue<br />

architecture might be of major<br />

importance. The underlying mechanisms<br />

needs to be elucidaded yet.<br />
