the Commission to reduce the filing fee from $350 to $125. As noted earlier herein, the<br />

registry is not meant to constitute a license, certification or warranty.<br />

Therefore, the Commission requires all <strong>CSP</strong> applicants to provide the following<br />

information <strong>with</strong> an application for registry:<br />

1. Completed <strong>Penn</strong>sylvania state tax certification statement.<br />

2. Disclosure of delinquency <strong>with</strong> taxing authorities in <strong>Penn</strong>sylvania.<br />

3. Disclosure of bankruptcy or liquidation proceedings for prior three years.<br />

4. Criminal disclosure (fraud, theft, larceny, deceit, consumer protection or<br />

deceptive trade law convictions or violations).<br />

5. A description ofthe types and amounts of insurance carried by the<br />

applicant.<br />

THEREFORE,<br />

IT IS ORDERED:<br />

1. That the Commission hereby establishes a Conservation Service<br />

Provider Registry pursuant to 66 Pa.C.S. § 2806.2.<br />

2. That the Commission hereby adopts the Application Form for Parties<br />

Wishing to Register <strong>as</strong> a Conservation Service Provider <strong>as</strong> found in Annex A.<br />

3. That the Commission hereby establishes a $125 initial application<br />

fee and a $25 application renewal fee.<br />

4. That the Commission's Bureau of Fixed Utility Services shall<br />

maintain and administer the registry, <strong>with</strong> the <strong>as</strong>sistance of the Commission's<br />
