Dear (Employee Name),<br />

It is our understanding that you are seeking <strong>as</strong>sistance through the Employee Assistance<br />

Program.<br />

Under the company Substance Abuse Program, in order to maintain your employment <strong>with</strong> the<br />

Company, it will be necessary for you to meet the folfowing conditions described below. Failure to<br />

do so will result in immediate termination of employment.<br />

1. You will be <strong>as</strong>sisted in making an appointment <strong>with</strong> the EAP for an initial evaluation.<br />

2. You will be placed on leave pending the results ofthe initial evaluation.<br />

3. When the EAP h<strong>as</strong> determined that you may return to work, you will be required to<br />

undergo a (insert either Retum-To-Duty Drug Test or Alcohol Test (see Retum to Duty<br />

Testing and Periodic Unannounced Follow-up Testing)) <strong>with</strong> negative test results.<br />

4. Upon retum to work (following rehabilitation treatment), you will be subject to Periodic<br />

Unannounced Follow-up testing for (insert either drugs or alcohol (see Retum to Duty<br />

Testing and Periodic Unannounced Follow-up Testing) <strong>as</strong> determined through consultation<br />

<strong>with</strong> the EAP. The frequency and duration of the Periodic Unannounced Follow-Up<br />

Testing are determined in consultation <strong>with</strong> the EAP.<br />

5. A positive test result or a refusal to test will result in immediate tennination of employment.<br />

6. In addition, you wil! be expected to maintain your attendance and performance in<br />

conformance <strong>with</strong> fhe Company's standards <strong>as</strong> well <strong>as</strong> abide by all the Company's usual<br />

requirements for continued employment.<br />
