PLAYERSfulfill the following requirements in order to qualify for an onsitemedical withdrawal:(a)(b)(c)(d)Meet the on-site PHCP and <strong>Tour</strong>nament Physician for anevaluation and assessment of the injury or illness prior tothe player leaving the <strong>Tour</strong>nament city;Submit a copy of a signed <strong>WTA</strong> <strong>Tour</strong> Medical InformationForm and Withdrawal Form to the on-site Supervisor,stating the reason for withdrawal and authorizing the <strong>WTA</strong>to release such reason to the media and public;Complete the Mandatory Activities set forth in Section IVas scheduled by the <strong>WTA</strong>; and, if applicable,Complete a First Round Retirement Form with the PHCPand the Supervisor (in cases when a retirement hasoccurred prior to or during the 1st round).iii.<strong>The</strong> On-Site Exception rule shall not apply to Top 10 Playerswithdrawing from Premier Mandatory or Premier 5<strong>Tour</strong>naments. For Top 10 Player withdrawals, see theSuspension Rule in Section II.F.d. Consecutive Medical WithdrawalsA player’s withdrawal from consecutive <strong>Tour</strong>naments due to aBona Fide Injury or following an On-Site Medical Withdrawal, willbe considered as one (1) withdrawal, and the player will only berequired to pay one (1) Late Withdrawal fine or use one (1) MedicalWithdrawal for consecutive subsequent withdrawals, should theyoccur prior to the Qualifying Sign-In deadline (4:00 p.m. tournamentlocal time, the day prior to the start of Qualifying).5. Extraordinary CircumstancesIn an extreme personal emergency or under other extraordinary circumstances(e.g., a death in the family or a serious illness or life-threateningsituation for the player or her family), a player has the right toappeal a fine to the <strong>WTA</strong>, provided such appeal is filed within 21 daysfrom the date of notice. Such Extraordinary Circumstances must besubstantiated by documentation.39
