The key priorities <strong>for</strong> action include:1. Reduction in the gaps between actual and expected prevalence <strong>for</strong> the keyvascular diseases via:The implementation of NHS health checks, to ensure a high uptakefrom those who are most at risk and more unlikely to take up a healthcheck e.g. men, BME communities and low income groups. Targetedpromotions, out-reach services and case-finding especially in relationto hypertension should be part of this response.Investigate practice outliers with low levels of prevalence <strong>for</strong> the CHDand stroke registers2. Variation in per<strong>for</strong>mance across practices <strong>for</strong> treatment outcomes:Investigate per<strong>for</strong>mance <strong>for</strong> practice outliers starting with bloodpressure and cholesterol management3. Develop an on-going programme of health equity audits supported by theincorporation of a health equity element into all planned primary care/serviceaudits. E.g. medicines management audits, service reviews, improved ethnicitymonitoring across primary care/ community services.4. Put strategies in place to increase referrals to LRMS <strong>for</strong> those on practiceregisters and improve outcomes <strong>for</strong> patients from deprived areas.5. Develop and implement a self care strategy as part of the long term conditionsstrategy so there is a menu of quality assured options <strong>for</strong> all newly diagnosedvascular patients.73
