MEDICINE & HEALTH The New Injectable Facelift By Laura E. Skellchock, MD, FAAD HHow many of us swore that we were going to do something to improve our appearance, but because of all of the commitments in our lives, we put it off…and here we are making the same resolution for 2007 that we did for 2006? Maybe it’s actually a good thing we didn’t have time before, since there are several new ways to dramatically improve appearance without surgery. It’s called the “NON-SURGICAL FACELIFT.” This is the newest way to turn back the clock – and it doesn’t require several weeks off from work, the cost is much less than that of a face lift and you look younger without that wind tunnel effect. The sine qua non of facial aging is volume loss. Children have pudgy, round faces with convex surfaces; elderly faces are convex, with sunken angles in the temples, cheeks and chin. Volume loss is caused by bone loss and loss of subcutaneous fat. This is why yo-yo weight loss is so detrimental – it’s much easier (and more obvious) to lose weight in our face than it is to gain weight there. Loss of volume leads to sagging skin. Imagine a balloon that has lost half its air – it starts to shrivel up and have “wrinkles.” Another factor in sagging skin is loss of elasticity and the effect of gravity – our skin doesn’t grow, but it can stretch out. If we can maintain and restore volume in our face, we can maintain the “air in the balloon” and have a more youthful convex shape. Soft tissue augmentation will create this more youthful face without surgery. The FDA approved products which are used to do this include Sculptra ® , Radiesse ® , Restylane ® and Juvederm ® . An experienced physician will use a combination of these products, effectively “sculpting” 54 JANUARY 2007 the face – adding volume wherever it is to achieve the desired result. Want to diminish your nasolabial folds? Filling them up, adding more volume to an already “bottom heavy” face is passé. The state of the art way is to build up the upper face and cheeks, creating more volume for the skin to drape over, effectively diminishing the folds by pulling them up. How to get rid of those bags under your eyes, especially if you’ve already had a blepharoplasty? Plump up the cheekbone beneath with Radiesse ® , and fill the area under the eye with Restylane ® . Another attribute of these new fillers is that they last longer than the earlier generation fillers such as collagen.The procedure is quick, can be done in the doctor’s office and requires only local anesthetic. It is important to avoid aspirin and aspirin containing products, alcohol, NSAIA’S, large doses of Vitamin E or Omega-3-fatty acids for a week before to diminish the possibility of bruising.Taking Arnica Montana for several days before and after the procedure also helps with bruising.
