All applications must contain:<br />
� Organization name and address plus the name and telephone number of the contact person taking the lead on<br />
the application and proposed scope of work.<br />
� Purpose and Need – Summarize the goals and objectives of the project. Identify the problems or needs that will<br />
be addressed. Identify who or what constituency will benefit from this work.<br />
� Implementation – Summarize the plans and time frame for implementation of the project. Identify individual(s)<br />
who will be responsible for overseeing the program and briefly describe their qualifications.<br />
� Evaluation – <strong>State</strong> the criteria and procedures that will be used to measure the success of the project or program<br />
and relate this to the stated goals and objectives.<br />
� Budget – Provide detailed projected incomes and expenses, all sources of program funding, and the period<br />
(beginning and ending date) for which funds are requested.<br />
� Future Support – Identify plans for securing ongoing support for this program once the NWO funds are no<br />
longer available. Include anticipated future financial needs for the program and potential sources of funding.<br />
� Dissemination – <strong>State</strong> how you will share the success of your program with other teachers and schools.<br />
Mandatory dissemination strategies are described below.<br />
� An application must be signed by the appropriate administrator/president/director of the organization.<br />
� An electronic application form must be submitted via e-mail for review.<br />
<strong>REPORT</strong>S<br />
All recipients of the funded projects must provide NWO with an annual report that contains a minimum of summarized data<br />
including: contact information for all leaders and participants, summary of activities including dates and times of<br />
events/activities, summarized attendance data, and summarized session evaluations using the NWO session evaluation<br />
instruments. A final budget report including detailed itemization of all approved expenses must also be included. Up to<br />
5% of the funds can be re-allocated within the approved budget categories without written PRIOR consent, but these<br />
modifications must be explained in the annual budget report. The NWO Director as deemed necessary may request other<br />
data. The annual report is due 30 days after the completed event/activity. Costs will not be reimbursed until this report is<br />
submitted and approved.<br />
COST REIMBURSEMENT<br />
Approved costs expended between December 1, <strong>2007</strong> and June 30, 2008 of the funded period will be reimbursed<br />
by BGSU pending the approval of the annual project report as detailed above.<br />
DISSEMINATION EXPECTATIONS<br />
Recipients are also expected to present a brief summary of the impact of the project at the NWO Fall Executive<br />
Board Meeting and submit a proposal for a related presentation at the subsequent Northwest Ohio Symposium on<br />
Mathematics and Science Teaching.<br />
