RESULTS Hematological recovery to > 0.5 x 109/L neutrophils and > 20 x 109/L platelets occurred in 16 of 21 evaluable patients at a median of 28 and 50 days postautograft, respectively. During the initial phase of hematopoietic regeneration in these 16 patients, only Phi-negative cells were detected in 13, with predominantly phi-negative cells (76% to 94%) in the other 3. Graft failure occurred in 5 patients, 3 of whom were rescued by re-establishing chronic phase disease following infusion of unmanipulated reserve autologous cells (Table 1). Subsequent to the initial recovery with Phi-negative cells, some Phi-positive cells reappeared between 4 and 36 months post-autograft in all but one of the 13 patients in whom complete (morphological and cytogenetic) remission had been achieved (the remaining patient died in remission). Nine of these 12 patients were then treated with a-interferon 1-3 x 10 6 units/m 2 ,3-7 days/week. Three subsequently returned to complete remission, 3 developed increasing numbers of Phi-positive cells and 3 are still too early to evaluate. Sixteen patients remain alive and well, 9 in hematological remission (8 in Group 1), 6 to 62 months (median 26) post-autograft. Three patients (all in Group 1) continue in complete remission after treatment with a-interferon. COMMENT Cultured autografts from some patients with CML can result in a consistent and sustained restoration of Phi-negative hematopoiesis after intensive therapy. Strategies for improving the utility of this approach are being developed.
