7.2 BESOINS EN MATIÈRE DE PLANIFICATION FAMILIALELes femmes actuellement en union non utilisatrices <strong>de</strong> <strong>la</strong> contraception et qui ont déc<strong>la</strong>ré neplus vouloir d'enfants (limiter les naissances) ou qui ont déc<strong>la</strong>ré vouloir attendre <strong>de</strong>ux ans ou plusavant <strong>la</strong> naissance <strong>de</strong> l'enfant suivant (espacer les naissances), sont considérées comme ayant <strong>de</strong>sbesoins non satisfaits en matière <strong>de</strong> contraception et constituent <strong>la</strong> <strong>de</strong>man<strong>de</strong> potentielle totale enp<strong>la</strong>nification familiale.Le tableau 7.3 présente les estimations <strong>de</strong>s besoins non satisfaits et satisfaits en matière <strong>de</strong>contraception pour les femmes en union <strong>de</strong> 15-49 ans selon certaines caractéristiquessociodémographiques, ainsi que pour les femmes qui ne sont pas en union et pour l’ensemble <strong>de</strong>sfemmes. Rappelons que le niveau d'utilisation actuelle <strong>de</strong> <strong>la</strong> contraception chez les femmes en union aconnu une augmentation importante <strong>de</strong>puis 1997, passant <strong>de</strong> 19 % à 27 % à l’EDSMD-III pouratteindre 40 % à l’enquête actuelle. Corré<strong>la</strong>tivement, les besoins non satisfaits en matière <strong>de</strong>p<strong>la</strong>nification familiale parmi les femmes en union continuent <strong>de</strong> diminuer passant <strong>de</strong> 26 % en 1997 à24 % en 2003-2004 et à 19 % à l’enquête actuelle. On note peu d’écarts entre <strong>la</strong> proportion <strong>de</strong> femmesayant <strong>de</strong>s besoins pour espacer et <strong>la</strong> proportion <strong>de</strong> celles qui veulent limiter leur <strong>de</strong>scendance (10 %contre 9 %), <strong>de</strong> même que pour les femmes dont les besoins sont actuellement satisfaits (19 % contre21 %).Si les femmes en union ayant <strong>de</strong>s besoins non satisfaits en matière <strong>de</strong> contraception pouvaientles satisfaire, c'est-à-dire si elles utilisaient effectivement <strong>la</strong> contraception, <strong>la</strong> prévalence contraceptivepourrait atteindre 59 % (<strong>de</strong>man<strong>de</strong> potentielle totale) ; <strong>la</strong> <strong>de</strong>man<strong>de</strong> potentielle totale en matière <strong>de</strong>contraception parmi les femmes en union était estimée à 51 % lors <strong>de</strong> l’ENDS 1992, à 45 % à l’EDS1997 et à 51 % à l’EDSMD-III. Cette <strong>de</strong>man<strong>de</strong> potentielle totale en p<strong>la</strong>nification familiale (proportion<strong>de</strong> femmes ayant <strong>de</strong>s besoins non satisfaits auxquels s’ajoutent les femmes déjà utilisatrices) estorientée dans 29 % <strong>de</strong>s cas vers l’espacement <strong>de</strong>s naissances et dans 30 % <strong>de</strong>s cas, vers <strong>la</strong> limitation.Actuellement chez les femmes en union, 68 % <strong>de</strong> <strong>la</strong> <strong>de</strong>man<strong>de</strong> potentielle totale en p<strong>la</strong>nificationfamiliale est satisfaite à Madagascar alors qu’elle n’était que <strong>de</strong> 53 % en 2003-2004.Les besoins en matière <strong>de</strong> p<strong>la</strong>nification familiale varient selon les caractéristiquessociodémographiques. Selon l’âge, on constate que c’est dans le groupe d’âges 15-19 ans que <strong>la</strong>proportion <strong>de</strong>s besoins non satisfaits en p<strong>la</strong>nification familiale est <strong>la</strong> plus élevée (27 %), ces besoinsétant orientés essentiellement vers l’espacement <strong>de</strong>s naissances (26 %) : seulement 48 % <strong>de</strong> <strong>la</strong><strong>de</strong>man<strong>de</strong> potentielle totale <strong>de</strong> ces jeunes femmes est satisfaite. La proportion <strong>de</strong> femmes ayant <strong>de</strong>sbesoins non satisfaits diminue ensuite pour rester quasiment stable entre 20 et 34 ans, oscil<strong>la</strong>nt autour<strong>de</strong> 16 % pour augmenter légèrement et atteindre 22 % à 40-44 ans ; au-<strong>de</strong>là, elle diminue pour neconcerner que 17 % <strong>de</strong>s femmes <strong>de</strong> 45-49 ans. En outre, les résultats montrent que les besoins nonsatisfaits pour l’espacement sont plus élevés chez les femmes âgées <strong>de</strong> 15 à 29 ans que chez les plusâgées (26 % à 15-19 ans contre 8 % et moins à partir <strong>de</strong> 30-34 ans). À l’opposé, <strong>la</strong> proportion <strong>de</strong>sbesoins non satisfaits pour <strong>la</strong> limitation <strong>de</strong>s naissances augmente avec l’âge <strong>de</strong>s femmes passant <strong>de</strong>5 % à 25-29 ans à 20 % à 40-44 ans.Les résultats selon le milieu <strong>de</strong> rési<strong>de</strong>nce ne font pas apparaître d’écarts importants (18 % <strong>de</strong>besoins non satisfaits en urbain contre 19 % en rural). Par contre, <strong>la</strong> <strong>de</strong>man<strong>de</strong> potentielle totale enmatière <strong>de</strong> p<strong>la</strong>nification familiale est plus élevée en milieu urbain (72 %) qu'en milieu rural (56 %),mais 76 % <strong>de</strong> cette <strong>de</strong>man<strong>de</strong> est satisfaite en urbain, contre 66 % en rural. Au niveau <strong>de</strong>s régions, lesproportions <strong>de</strong> femmes ayant <strong>de</strong>s besoins non satisfaits en matière <strong>de</strong> p<strong>la</strong>nification familiale varientd’un minimum <strong>de</strong> 12 % dans les régions d’Ihorombe et d’Atsinanana à un maximum <strong>de</strong> 27 % danscelle <strong>de</strong> Vatovavy Fitovinany. Par ailleurs, on note que <strong>la</strong> région d’Ana<strong>la</strong>manga présente <strong>la</strong> <strong>de</strong>man<strong>de</strong>potentielle totale <strong>la</strong> plus élevée (76 %, mais 76 % <strong>de</strong> cette <strong>de</strong>man<strong>de</strong> est satisfaite), alors qu’avec 27 %,<strong>la</strong> région d’Androy enregistre une <strong>de</strong>man<strong>de</strong> potentielle totale particulièrement faible, dont seulement16 % est satisfaite.Préférences en matière <strong>de</strong> fécondité | 119

Les besoins non satisfaits pour limiter concernent les femmesenceintes dont <strong>la</strong> grossesse n'était pas souhaitée, les femmes en aménorrhée qui n'utilisent pas <strong>de</strong> métho<strong>de</strong> <strong>de</strong> p<strong>la</strong>nification familiale dont <strong>la</strong><strong>de</strong>rnière naissance n'était pas voulue et qui ne veulent plus d'enfants et les femmes fécon<strong>de</strong>s qui ne sont, ni enceintes ni en aménorrhée, quin'utilisent pas <strong>de</strong> métho<strong>de</strong> <strong>de</strong> p<strong>la</strong>nification familiale et qui ont déc<strong>la</strong>ré ne plus vouloir d'enfants.2L'utilisation pour espacer concerne les femmes qui utilisent actuellement une métho<strong>de</strong> contraceptive et qui ont déc<strong>la</strong>ré vouloir un autre enfantou qui ne sont pas sûres d'en vouloir un autre.
