APPLICATION MATERIALS AND GUIDELINES<br />

Submit materials to your Dean in time for review and submission to the Provost by December 1.<br />

Note differences in the guidelines depending on the year you were hired: before July 1, 1999 or<br />

after July 1, 1999. If you are applying for both promotion and tenure, you need to prepare only one<br />

portfolio. Be sure to include application materials for both.<br />

Portfolios must be no larger than a four inch binder.<br />

The outside of your portfolio should have, at a minimum, your name and the action(s)<br />

[promotion, tenure, or promotion and tenure] you seek.<br />

Create a Table of Contents.<br />

Use tabs to separate sections.<br />

Because so many people must handle your portfolio, consider using sleeves for each page<br />

for protection and durability.<br />

Evaluation from Dean, which must include in-class observation. In-class observations must<br />

document the date and time of the observation, the course name, meeting place, and<br />

number of students in attendance.<br />

Evaluations from Peers: An evaluation of the faculty member’s performance by a colleague<br />

designated by the Dean, which must include in-class observation. In-class observations<br />

must document the date and time of the observation, the course name, meeting place, and<br />

number of students in attendance.<br />

If you are applying for promotion only, you will have two observations and evaluations<br />

from peers and one evaluation from your dean. Note that all evaluations are to include<br />

classroom observation, with specific information about the class observed to be included.<br />

If you are applying for tenure only, you will have two observations and evaluations from<br />

peers and one evaluation from your dean. Note that all evaluations are to include classroom<br />

observation, with specific information about the class observed to be included.<br />

If you are applying for promotion and tenure, you will have four evaluations and<br />

observations from peers (two for promotion and two for tenure). If a candidate is not being<br />

recommended for tenure, the dean would not recommend that candidate for promotion. If,<br />

however, the dean recommends for tenure, he/she may or may not recommend for<br />

promotion as well.<br />

If you wish to use out-of-field observers, please discuss this with your dean.<br />

Materials for the packet are available in the Forms Repository online. The evaluations are sent<br />

directly to the Provost. The Personnel Committee will insert them into your portfolio.<br />

2. Vita: The vita should be organized in the following format:<br />

A list of degrees attained beginning with the highest degree. Indicate institution, field of<br />

study, degrees attained and date. (Official transcripts of all education credentials must be in
