New <strong>Faculty</strong> Enrollment Period for PEIA Health and Life Insurances and Mountaineer Flexible<br />

Benefit Plans<br />

New hires have the month of hire and the two following months to enroll in health, basic, optional<br />

and dependent life, and Mountaineer Flexible Benefit plans. Documentation for dependents is<br />

required for health coverage to be approved (marriage license/spouse; birth certificate/children).<br />

Effective Date of Coverage:<br />

Coverage is effective the first of the month following the month of enrollment in the plan. Example:<br />

if you are hired in August and enroll, coverage is effective September 1.<br />

HEALTH INSURANCE<br />

The WV Public Employees Insurance Agency (PEIA) offers a choice of plans of a preferred provider<br />

benefit plan (PPB) and managed care plans (HMO) for employees and their dependents. Employee<br />

premiums are based on annual base salary, coverage type (single or family), and the plan selected<br />

(see discounts below). New hires have the month of hire and the following two months to select<br />

coverage. Coverage is effective the first of the month following the month of enrollment. Premiums<br />

are taken on a pre-tax basis. PEIA requires a copy of the marriage certificate to add a spouse, and a<br />

copy of the birth certificate for each dependent child from birth up to age 26.<br />

In April, PEIA mails to each employee a “Shopper’s Guide”, which outlines the benefits for the<br />

plans offered, and premiums for both health and optional life insurances for the plan year. In<br />

addition, refer to the Summary Plan Description booklet for more information. Both booklets are<br />

available on the PEIA web site: www.wvpeia.com<br />

PEIA PPB PLANS A, B, D<br />

Preferred Provider Benefit Plan premiums for this plan are based on employee and dependent<br />

Tobacco Status. Premiums, annual deductibles, and co-insurance amounts are based on the<br />

employee’s annual salary. Plan D is for WV residents who received all care in WV, except<br />

emergencies and approved procedures not available in WV.<br />

There are restrictions on traveling outside West Virginia to get medical care. Prescription drugs<br />

have a separate plan deductible and out-of-pocket maximum.<br />

PEIA PPB PLAN C<br />

A High Deductible Health Plan, which can be paired with a Health Savings Account or a Health<br />

Reimbursement Arrangement. Single deductible is $1250; children or family $2500. Premiums are<br />

based on tobacco status, but not on salary.<br />

PEIA MANAGED CARE PLANS (HMO)<br />

HMO’s manage health care for their members by coordinating the use of health care services<br />

through Primary Care Physicians. PEIA offers HMO plans with “The Health Plan”. Members must be<br />

referred by the PCP for specialty care within the network. Family members must live in the plan<br />

enrollment area. Premiums are based on salary, tobacco status and living will status, and are set by<br />
