the plann<strong>in</strong>g process will confront the difficult questions<br />

<strong>of</strong> f<strong>in</strong>anc<strong>in</strong>g, partnerships and coord<strong>in</strong>at<strong>in</strong>g responsibilities,<br />

<strong>in</strong>clud<strong>in</strong>g the harmonization <strong>of</strong> donor activities on the basis<br />

<strong>of</strong> the national strategy.<br />

2.3.1. Structure <strong>of</strong> the strategy<br />

While plann<strong>in</strong>g the structure <strong>of</strong> the strategy, four vertical<br />

and three horizontal approaches were taken <strong>in</strong>to consideration.<br />

Vertical approaches <strong>in</strong>clude:<br />

• <strong>health</strong> <strong>in</strong> all policies: government <strong>policy</strong> support<strong>in</strong>g<br />

<strong>health</strong> and <strong>in</strong>tersectoral cooperation <strong>in</strong> <strong>health</strong> promotion<br />

and disease prevention issues, <strong>in</strong>clud<strong>in</strong>g:<br />

--<br />

legislation;<br />

--<br />

coord<strong>in</strong>ation and management <strong>of</strong> the strategy; and<br />

--<br />

mechanisms <strong>of</strong> f<strong>in</strong>anc<strong>in</strong>g;<br />

• <strong>health</strong>y lifestyles and risk factor reduction: boost<strong>in</strong>g personal<br />

<strong>in</strong>volvement <strong>of</strong> the population to support <strong>health</strong>y<br />

lifestyles, reduce risk factors and develop personal skills<br />

for <strong>health</strong>ier choices, <strong>in</strong>clud<strong>in</strong>g:<br />

--<br />

<strong>development</strong> <strong>of</strong> <strong>in</strong>dividual/personal skills and abilities<br />

to live a <strong>health</strong>y lifestyle;<br />

--<br />

fight<strong>in</strong>g harmful habits; and<br />

--<br />

creat<strong>in</strong>g the conditions that enable people to make<br />

<strong>health</strong>y choices <strong>in</strong> everyday life;<br />

• partnerships: support<strong>in</strong>g <strong>health</strong> <strong>in</strong> everyday life at different<br />

levels and sett<strong>in</strong>gs <strong>of</strong> the community, <strong>in</strong>clud<strong>in</strong>g:<br />

--<br />

broaden<strong>in</strong>g <strong>health</strong> promotion activities <strong>in</strong> everyday<br />

sett<strong>in</strong>gs;<br />

--<br />

creat<strong>in</strong>g partnerships <strong>in</strong> support <strong>of</strong> <strong>health</strong> at all<br />

levels; and<br />

--<br />

strengthen<strong>in</strong>g the strategic character <strong>of</strong> <strong>in</strong>ternational<br />

cooperation;<br />

• <strong>health</strong> services: to strengthen the preventive work <strong>of</strong><br />

medical organizations and <strong>in</strong>stitutions, <strong>in</strong>clud<strong>in</strong>g:<br />

--<br />

strengthen<strong>in</strong>g and develop<strong>in</strong>g <strong>health</strong> promotion and<br />

NCD prevention <strong>in</strong> public <strong>health</strong> <strong>in</strong>stitutions;<br />

--<br />

<strong>in</strong>creas<strong>in</strong>g the role <strong>of</strong> PHC <strong>in</strong> NCD prevention and<br />

<strong>health</strong> promotion; and<br />

--<br />

develop<strong>in</strong>g cooperation with the cl<strong>in</strong>ical sector.<br />

Horizontal approaches comprise:<br />

• reduc<strong>in</strong>g <strong>in</strong>equities and hav<strong>in</strong>g a positive effect on the<br />

socioeconomic determ<strong>in</strong>ants <strong>of</strong> <strong>health</strong>:<br />

--<br />

a positive effect on socioeconomic determ<strong>in</strong>ants and<br />

an improvement <strong>in</strong> the <strong>health</strong> <strong>of</strong> the overall population;<br />

--<br />

a reduction <strong>in</strong> <strong>health</strong> <strong>in</strong>equities as an <strong>in</strong>tegral part <strong>of</strong><br />

the plann<strong>in</strong>g, implementation and evaluation <strong>of</strong> any<br />

action with<strong>in</strong> the framework <strong>of</strong> the strategy; and<br />

--<br />

the <strong>development</strong> <strong>of</strong> special actions aimed at vulnerable<br />

groups;<br />

• <strong>health</strong> communication, target<strong>in</strong>g:<br />

--<br />

various groups <strong>of</strong> the population;<br />

--<br />

medical workers; and<br />

--<br />

other sectors and partners;<br />

• monitor<strong>in</strong>g, evaluation and effectiveness <strong>of</strong> ongo<strong>in</strong>g<br />

measures:<br />

--<br />

monitor<strong>in</strong>g and evaluation <strong>of</strong> strategy implementation<br />

as a whole, tak<strong>in</strong>g <strong>in</strong>to consideration processes<br />

and results, and<br />

--<br />

separate monitor<strong>in</strong>g and evaluation <strong>of</strong> each step.<br />

Concrete measures are <strong>in</strong>tegrated <strong>in</strong>to this plann<strong>in</strong>g framework<br />

and are essentially <strong>in</strong>terrelated. The adoption <strong>of</strong> the<br />

document <strong>in</strong> July 2007 at a national workshop also served<br />

as a basis for elaborat<strong>in</strong>g the concrete steps <strong>in</strong> its implementation.<br />

2.4. Key <strong>in</strong>stitutions <strong>in</strong> the field <strong>of</strong> public <strong>health</strong> and<br />

<strong>health</strong> promotion<br />

Kyrgyzstan faces the challenge <strong>of</strong> new ways <strong>of</strong> governance.<br />

The whole public adm<strong>in</strong>istration system is chang<strong>in</strong>g. Intersectoral<br />
