effectiveness <strong>of</strong> <strong>in</strong>terventions. A strong focus on review<strong>in</strong>g<br />

and evaluat<strong>in</strong>g NCD policies, with the strengthen<strong>in</strong>g<br />

<strong>of</strong> <strong>in</strong>tervention and implementation research, would<br />

greatly enhance knowledge on what works and what<br />

does not, particularly <strong>in</strong> build<strong>in</strong>g partnerships for NCD<br />

<strong>policy</strong>.<br />

• Fund<strong>in</strong>g for evaluation. In the plann<strong>in</strong>g phase <strong>of</strong> policies<br />

and programmes, an appropriate level <strong>of</strong> fund<strong>in</strong>g should<br />

be guaranteed with<strong>in</strong> public adm<strong>in</strong>istration to fund monitor<strong>in</strong>g<br />

and evaluation. In addition, the research community<br />

should be encouraged by designated fund<strong>in</strong>g to help<br />

fill gaps on what does and what does not work.<br />

• Shar<strong>in</strong>g the knowledge <strong>of</strong> what works <strong>in</strong>ternationally. There<br />

has been an expansion <strong>of</strong> “best practice” <strong>in</strong>formation<br />

<strong>in</strong> recent years but this may not be easily accessible.<br />

There is a need for new frames <strong>of</strong> reference for better<br />

understand<strong>in</strong>g and more developed methodologies <strong>of</strong><br />

adaptation to local needs.<br />

• Transfer <strong>of</strong> knowledge with<strong>in</strong> countries. Most countries<br />

have examples <strong>of</strong> <strong>in</strong>novative action or pilot projects<br />

at the subnational level, but this is not always widely<br />

known or used. Stronger channels for shar<strong>in</strong>g positive<br />

and negative experiences and for discuss<strong>in</strong>g what went<br />

right or wrong could help the dissem<strong>in</strong>ation <strong>of</strong> exist<strong>in</strong>g<br />

good practices with<strong>in</strong> countries. Successful practices and<br />

pilots should be better <strong>in</strong>tegrated <strong>in</strong>to the overall <strong>policy</strong><br />

<strong>development</strong> process.<br />

• Capacity build<strong>in</strong>g <strong>in</strong> m<strong>in</strong>istries <strong>of</strong> <strong>health</strong>. The capacity<br />

<strong>of</strong> the m<strong>in</strong>istry <strong>of</strong> <strong>health</strong> to meet the needs for NCD<br />

<strong>policy</strong> <strong>development</strong>, and <strong>of</strong> public adm<strong>in</strong>istration <strong>in</strong><br />

general, is rather weak <strong>in</strong> some countries. It could be<br />

enhanced by tra<strong>in</strong><strong>in</strong>g, <strong>in</strong> relation not only to tackl<strong>in</strong>g<br />

NCD but also to <strong>in</strong>novative and flexible <strong>policy</strong>-mak<strong>in</strong>g<br />

and plann<strong>in</strong>g, modern public <strong>health</strong> approaches and the<br />

skills for <strong>in</strong>tersectoral cooperation.<br />

• Ensur<strong>in</strong>g the right care, at the right time, <strong>in</strong> the right place.<br />

Access to prevention and <strong>health</strong> promotion services,<br />

and to high-quality care and rehabilitation for NCD, and<br />

the results <strong>of</strong> such care, present a challenge for achiev<strong>in</strong>g<br />

equity <strong>in</strong> <strong>health</strong>. Review <strong>of</strong> the quality, appropriateness<br />

and timel<strong>in</strong>ess <strong>of</strong> care for NCD is needed to ensure that<br />

effective <strong>in</strong>terventions are given priority. Evidence-based<br />

guidel<strong>in</strong>es are available <strong>in</strong> some countries, and their<br />

wider adoption and implementation could be facilitated<br />

through performance monitor<strong>in</strong>g, fund<strong>in</strong>g mechanisms<br />

and <strong>in</strong>centive schemes.<br />

• Address<strong>in</strong>g long-term needs. Those who suffer from<br />

NCD, and their families, may live with the consequences<br />

<strong>of</strong> these diseases for many years. The way <strong>in</strong> which<br />

exist<strong>in</strong>g <strong>health</strong> and social services meet the long-term<br />

needs <strong>of</strong> patients and carers can be assessed, with a<br />

view to develop<strong>in</strong>g more <strong>in</strong>tegrated, patient-centred<br />

care closer to home. It may be worth consider<strong>in</strong>g test<strong>in</strong>g<br />

and adapt<strong>in</strong>g <strong>in</strong>novations developed <strong>in</strong> certa<strong>in</strong> countries.<br />

Establish<strong>in</strong>g a unify<strong>in</strong>g framework<br />

Many countries have a comb<strong>in</strong>ation <strong>of</strong> broad policies for<br />

<strong>health</strong> (and/or policies for <strong>health</strong> promotion and <strong>health</strong><br />

care), together with policies for specific NCD and/or their<br />

risk factors. In no case, among the countries studied, does<br />

there appear to be clear coord<strong>in</strong>ation or harmonization <strong>of</strong><br />

these various efforts. Even with<strong>in</strong> broad <strong>health</strong> promotion<br />

policies, <strong>in</strong>dividual NCD or <strong>health</strong> risks appear to be tackled<br />

<strong>in</strong> a vertical way. The <strong>European</strong> NCD Strategy provides<br />

the opportunity to establish a unify<strong>in</strong>g action framework or<br />

“umbrella” that draws together the <strong>in</strong>dividual components<br />

towards a common goal.<br />

• Understand<strong>in</strong>g the situation. A comprehensive situation<br />

<strong>analysis</strong>, both <strong>of</strong> the burden <strong>of</strong> NCD and the exist<strong>in</strong>g<br />

policies, strategies and actions for tackl<strong>in</strong>g them, would<br />

give a better understand<strong>in</strong>g <strong>of</strong> the challenges and exist<strong>in</strong>g<br />

gaps.<br />

• Mapp<strong>in</strong>g a way forward. To support coord<strong>in</strong>ation among<br />

partners, it is essential to clearly state and dissem<strong>in</strong>ate<br />

widely:<br />
