• Utiliz<strong>in</strong>g programmes for poverty and social exclusion.<br />
Programmes to tackle poverty and social exclusion <strong>of</strong>fer<br />
opportunities for elaborat<strong>in</strong>g the synergies between<br />
NCD and <strong>development</strong>, and <strong>in</strong>dicat<strong>in</strong>g the potential<br />
benefits <strong>in</strong> both <strong>health</strong> and economic terms.<br />
• Mak<strong>in</strong>g <strong>health</strong> an <strong>in</strong>dicator <strong>of</strong> <strong>development</strong>. Report<strong>in</strong>g on<br />
changes <strong>in</strong> <strong>in</strong>equalities <strong>in</strong> NCD, dur<strong>in</strong>g annual budget<br />
discussions, could help ensure that <strong>health</strong> is <strong>in</strong>cluded<br />
<strong>in</strong> the concept <strong>of</strong> <strong>development</strong>. Health, <strong>in</strong>clud<strong>in</strong>g the<br />
prevention and control <strong>of</strong> NCD, can contribute to<br />
poverty reduction and economic <strong>development</strong>. Given<br />
this, several countries have adapted the Millennium<br />
Development Goal 6, (on combat<strong>in</strong>g HIV/AIDS, malaria<br />
and other diseases) to <strong>in</strong>clude chronic diseases or NCD<br />
(22).<br />
• Equity-oriented HIA. The WHO def<strong>in</strong>ition <strong>of</strong> HIA (23)<br />
expla<strong>in</strong>s that, <strong>in</strong> assess<strong>in</strong>g the impact <strong>of</strong> policies <strong>in</strong> sectors<br />
other than <strong>health</strong>, their “potential effects on the<br />
<strong>health</strong> <strong>of</strong> a population and the distribution <strong>of</strong> those<br />
effects with<strong>in</strong> the population” need to be judged. This<br />
equity aspect is rarely implemented.<br />
• Improved management <strong>of</strong> care for NCD. Not only the<br />
distribution but also the management <strong>of</strong> care for NCD<br />
must be improved if access to high-quality and appropriate<br />
care is to be ensured, at the right time and <strong>in</strong> the<br />
right place. Better understand<strong>in</strong>g <strong>of</strong> the drivers <strong>of</strong> <strong>health</strong><br />
<strong>in</strong>equalities, and modell<strong>in</strong>g the specific <strong>in</strong>terventions<br />
needed to reduce these, has also shown that greater<br />
access to cheap and effective treatment (such as the<br />
management <strong>of</strong> hypertension) could play a significant<br />
role <strong>in</strong> narrow<strong>in</strong>g the gap between those liv<strong>in</strong>g <strong>in</strong> the<br />
most deprived and the least deprived areas (24).<br />
• Examples <strong>of</strong> good practice. A compilation <strong>of</strong> well-evaluated<br />
examples <strong>of</strong> good practice would enable countries<br />
to consider <strong>in</strong>novative action and different options for<br />
tackl<strong>in</strong>g <strong>in</strong>equalities <strong>in</strong> NCD.<br />
Government decision-mak<strong>in</strong>g should take place <strong>in</strong> a<br />
transparent manner<br />
Tackl<strong>in</strong>g NCD requires tak<strong>in</strong>g difficult decisions, some <strong>of</strong><br />
which may be <strong>in</strong> conflict with the <strong>in</strong>terests <strong>of</strong> certa<strong>in</strong> groups<br />
or may be negatively received by the public. This, comb<strong>in</strong>ed<br />
with the need to <strong>in</strong>volve multiple partners, means that particular<br />
care is needed to ensure that government decisionmak<strong>in</strong>g<br />
takes place <strong>in</strong> a transparent manner.<br />
• Time and resources. Broad and open consultations can<br />
take place only if sufficient time and resources are designated<br />
for this purpose.<br />
• Effective participation. There will be confidence <strong>in</strong> the<br />
process only if adequate attention is paid to feedback,<br />
mak<strong>in</strong>g it clear that comments, suggestions and op<strong>in</strong>ions<br />
<strong>of</strong>fered are taken <strong>in</strong>to account.<br />
• Strengthen<strong>in</strong>g civil society. Civil society, NGOs and<br />
consumer societies have an outstand<strong>in</strong>g role to play <strong>in</strong><br />
hold<strong>in</strong>g government and the private sector responsible<br />
for their actions and act<strong>in</strong>g as a watchdog on <strong>policy</strong><br />
implementation. In some countries, they may need tra<strong>in</strong><strong>in</strong>g,<br />
capacity build<strong>in</strong>g and support, <strong>in</strong>clud<strong>in</strong>g <strong>in</strong>formation<br />
and fund<strong>in</strong>g, <strong>in</strong> order to carry out this role adequately.<br />
• Transparency <strong>in</strong> spend<strong>in</strong>g the money. Transparency <strong>in</strong> the<br />
allocation <strong>of</strong> fund<strong>in</strong>g is <strong>of</strong> outstand<strong>in</strong>g importance. The<br />
criteria for fund<strong>in</strong>g, <strong>in</strong>clud<strong>in</strong>g the fund<strong>in</strong>g <strong>of</strong> research,<br />
must be clear and the priorities <strong>in</strong> l<strong>in</strong>e with NCD <strong>policy</strong>.<br />
Countries can build on exist<strong>in</strong>g experience and move<br />
forward from whatever po<strong>in</strong>t they have reached<br />
All countries have some experience <strong>in</strong> develop<strong>in</strong>g broad or<br />
issue-specific policies for NCD. Furthermore, comparative<br />
newcomers can learn from those with longer experience –<br />
there is no need to re<strong>in</strong>vent the wheel.<br />
• Evaluat<strong>in</strong>g actions. To build on what a country has, it<br />
needs to know what exists already and whether it<br />
works. Evaluation <strong>of</strong> NCD policies (as opposed to monitor<strong>in</strong>g)<br />
is comparatively weak, as is evaluation <strong>of</strong> the<br />
