tation, and an important <strong>in</strong>vestment, the experience and<br />

lessons learnt were never really used on a broader scale.<br />

The North Karelia project <strong>in</strong> F<strong>in</strong>land cont<strong>in</strong>ues, however,<br />

to perform a pilot function for certa<strong>in</strong> issues, even after so<br />

many years <strong>of</strong> operation. The CINDI programme, particularly<br />

as implemented <strong>in</strong> Lithuania, has also <strong>in</strong>dicated the<br />

value <strong>of</strong> test<strong>in</strong>g complex NCD <strong>in</strong>terventions on a small<br />

scale through a demonstration site before expand<strong>in</strong>g or<br />

generaliz<strong>in</strong>g the approach.<br />

Monitor<strong>in</strong>g and evaluation<br />

By monitor<strong>in</strong>g we mean surveillance and record<strong>in</strong>g <strong>of</strong><br />

observed changes, be they changes <strong>in</strong> epidemiological data,<br />

steps taken towards adm<strong>in</strong>istrative or legislative action, or<br />

funds expended. Evaluation goes a step further, assess<strong>in</strong>g<br />

whether changes are <strong>in</strong> the right direction and if <strong>policy</strong> proposals<br />

are be<strong>in</strong>g achieved <strong>in</strong> the sense <strong>of</strong> what went right or<br />

wrong, and why and how.<br />

As might be expected from the mixed picture <strong>of</strong> implementation,<br />

the situation regard<strong>in</strong>g the monitor<strong>in</strong>g and evaluation<br />

<strong>of</strong> NCD <strong>policy</strong> is equally mixed and rather less vibrant<br />

than that <strong>of</strong> <strong>policy</strong> formulation. At one extreme, ow<strong>in</strong>g to<br />

the <strong>in</strong>itial stages <strong>of</strong> the <strong>policy</strong> <strong>development</strong> process as <strong>in</strong><br />

Greece, the newness <strong>of</strong> the <strong>policy</strong> as <strong>in</strong> Kyrgyzstan or the<br />

failure to implement as <strong>in</strong> Albania, there is little yet to monitor<br />

<strong>in</strong> <strong>policy</strong> terms. At the other extreme, <strong>in</strong> countries with<br />

well-developed databases, a long history <strong>of</strong> well-function<strong>in</strong>g<br />

surveillance systems and strong research <strong>in</strong>stitutions, such<br />

as F<strong>in</strong>land, Ireland and Lithuania there is a well-developed<br />

culture <strong>of</strong> monitor<strong>in</strong>g and evaluat<strong>in</strong>g <strong>health</strong> <strong>policy</strong>. These<br />

countries have a tradition <strong>of</strong> transparency <strong>in</strong> monitor<strong>in</strong>g<br />

progress <strong>in</strong> <strong>policy</strong> implementation and <strong>of</strong> report<strong>in</strong>g back,<br />

frequently to parliament.<br />

Approach<br />

Changes <strong>in</strong> mortality rates are those most frequently<br />

monitored, followed by changes <strong>in</strong> behaviour where this is<br />

regularly surveyed. The impact <strong>of</strong> <strong>health</strong> promotion and the<br />

uptake <strong>of</strong> screen<strong>in</strong>g programmes are evaluated <strong>in</strong> four <strong>of</strong><br />

the countries exam<strong>in</strong>ed. There is less emphasis on monitor<strong>in</strong>g<br />

the function<strong>in</strong>g <strong>of</strong> the <strong>health</strong> system or the coord<strong>in</strong>ation<br />

<strong>of</strong> <strong>policy</strong> implementation, although F<strong>in</strong>land and to some<br />

extent Hungary try to do this.<br />

In F<strong>in</strong>land, where there has been rout<strong>in</strong>e surveillance <strong>of</strong><br />

chronic diseases and their risk factors s<strong>in</strong>ce the early 1970s,<br />

a process has recently been <strong>in</strong>stituted whereby each m<strong>in</strong>istry<br />

and level <strong>of</strong> government is obliged to report back on its<br />

impact on <strong>health</strong>, thus contribut<strong>in</strong>g a strong <strong>in</strong>tersectoral<br />

aspect to the overall report to parliament on <strong>health</strong> <strong>policy</strong><br />

implementation. In Lithuania, the National Board <strong>of</strong> Health<br />

reports annually directly to Parliament. Significantly <strong>in</strong> this<br />

case, the Board enjoys a great deal <strong>of</strong> <strong>in</strong>dependence from<br />

the M<strong>in</strong>istry <strong>of</strong> Health.<br />

There has been annual report<strong>in</strong>g <strong>of</strong> progress <strong>in</strong> <strong>policy</strong><br />

<strong>development</strong> <strong>in</strong> Ireland s<strong>in</strong>ce 2001. Different structures and<br />

processes for monitor<strong>in</strong>g and evaluation have been tried,<br />

<strong>in</strong>clud<strong>in</strong>g a high-level Cab<strong>in</strong>et Subcommittee (which was<br />

not so successful), <strong>in</strong>terdepartmental group reviews and<br />

reports, the regular report<strong>in</strong>g <strong>of</strong> the National Implementation<br />

Team; and reports <strong>of</strong> specially constituted task forces<br />

report<strong>in</strong>g on issue-specific policies.<br />

One <strong>of</strong> the encourag<strong>in</strong>g <strong>development</strong>s is that, where a<br />

system <strong>of</strong> monitor<strong>in</strong>g and evaluation is <strong>in</strong> place, this is usually<br />

reflected <strong>in</strong> published documents <strong>in</strong> a transparent and<br />

accessible way. The Internet <strong>of</strong>fers a new possibility for a<br />

broad accessibility <strong>of</strong> published monitor<strong>in</strong>g and evaluation<br />

results. In Hungary, for <strong>in</strong>stance, some public <strong>health</strong> reports<br />

and other monitor<strong>in</strong>g results <strong>of</strong> the National Public Health<br />

Strategy are available on the Internet, although so far without<br />

special attention to their <strong>in</strong>terest for the general public.<br />

In F<strong>in</strong>land, France, Ireland and Lithuania, <strong>policy</strong> documents<br />

stated that an <strong>in</strong>terim review should be carried out part-<br />
