disease prevention at government level are at the plann<strong>in</strong>g<br />

stage.<br />

Currently, as <strong>in</strong> other central Asian countries, the role <strong>of</strong><br />

the Kyrgyz M<strong>in</strong>istry <strong>of</strong> Health is mostly adm<strong>in</strong>istrative, managerial<br />

and regulatory. It directly adm<strong>in</strong>isters government<br />

<strong>health</strong> facilities such as the scientific research <strong>in</strong>stitutes and<br />

national centres <strong>of</strong> excellence and manages the Kyrgyz State<br />

Medical Academy. Until recently, the Medical Academy was<br />

unable to control the number <strong>of</strong> admissions. It appo<strong>in</strong>ts the<br />

heads <strong>of</strong> government-based <strong>health</strong> care organizations and<br />

its prior agreement has to be obta<strong>in</strong>ed to appo<strong>in</strong>t heads <strong>of</strong><br />

municipal <strong>health</strong> organizations. The overwhelm<strong>in</strong>g role <strong>of</strong><br />

academic bodies is a rather conservative and an obstruction<br />

to <strong>health</strong> <strong>policy</strong> <strong>development</strong>.<br />

S<strong>in</strong>ce donors play an important role <strong>in</strong> Kyrgyzstan, the<br />

M<strong>in</strong>istry <strong>of</strong> Health is also responsible for coord<strong>in</strong>at<strong>in</strong>g<br />

relevant donor activities and distribut<strong>in</strong>g <strong>of</strong> humanitarian<br />

aid. This very complicated and sensitive task is not carried<br />

out <strong>in</strong> a satisfactory way at the moment. At least <strong>in</strong> the field<br />

<strong>of</strong> public <strong>health</strong> and <strong>health</strong> promotion, most activities are<br />

driven by external donors and the M<strong>in</strong>istry <strong>of</strong> Health has<br />

rather limited <strong>in</strong>fluence on the support <strong>of</strong> the <strong>in</strong>ternational<br />

community.<br />

The M<strong>in</strong>istry <strong>of</strong> Health develops a <strong>health</strong> budget accord<strong>in</strong>g<br />

to national <strong>health</strong> <strong>policy</strong> priorities, tak<strong>in</strong>g <strong>in</strong>to account<br />

donors and <strong>health</strong> revenue estimates. The budget plann<strong>in</strong>g<br />

process is a sensitive issue <strong>in</strong> Kyrgyzstan, <strong>in</strong> which the M<strong>in</strong>istry<br />

<strong>of</strong> F<strong>in</strong>ance also plays a crucial role. The basic problem<br />

is the poor economic <strong>development</strong> <strong>of</strong> the country, and the<br />

budgetary possibilities <strong>of</strong> f<strong>in</strong>anc<strong>in</strong>g <strong>health</strong> care are rather<br />

limited and <strong>in</strong>flexible. In the face <strong>of</strong> serious underf<strong>in</strong>anc<strong>in</strong>g,<br />

one <strong>of</strong> the specific dilemmas <strong>of</strong> the Kyrgyz situation is<br />

how to <strong>in</strong>tegrate strategic considerations <strong>of</strong> <strong>health</strong> promotion<br />

and NCD prevention to a <strong>policy</strong> basically under crisis<br />

management. If successfully implemented, Manas Taalimi<br />

may serve as a good example <strong>of</strong> mov<strong>in</strong>g from <strong>in</strong>cremental,<br />

emergency-type action to long-term, susta<strong>in</strong>able <strong>policy</strong><br />

<strong>development</strong> <strong>in</strong> the field <strong>of</strong> <strong>health</strong>.<br />

Local state adm<strong>in</strong>istrations are responsible for <strong>health</strong><br />

plann<strong>in</strong>g, regulation, coord<strong>in</strong>ation and implementation at<br />

subnational level. Prior to major local reforms <strong>in</strong> 2000,<br />

these functions were performed by oblast <strong>health</strong> departments.<br />

As the result <strong>of</strong> the reforms, these departments<br />

were abolished and their functions transferred to regional<br />

hospitals and then to the supervisory councils for <strong>health</strong><br />

management, which became the coord<strong>in</strong>ation commissions<br />

on <strong>health</strong> management <strong>in</strong> 2003.<br />

Accord<strong>in</strong>g to Kyrgyz law, NGOs (at least theoretically)<br />

“take part <strong>in</strong> ensur<strong>in</strong>g <strong>health</strong> security and promotion and<br />

provide scientific and <strong>health</strong> <strong>in</strong>formation” (3). A large<br />

number <strong>of</strong> NGOs are active, <strong>in</strong>clud<strong>in</strong>g <strong>in</strong>itiative groups and<br />

<strong>health</strong> committees and <strong>in</strong>ternational agencies such as the<br />

Soros Foundation–Kyrgyzstan and Counterpart Consortium,<br />

along with projects funded by <strong>in</strong>ternational organizations<br />

to support the <strong>development</strong> <strong>of</strong> civil society <strong>in</strong>itiatives.<br />

2.4.1. Primary <strong>health</strong> care<br />

The PHC system <strong>in</strong>cludes various levels and entry po<strong>in</strong>ts for<br />

patients:<br />

• feldsher-obstetrical posts<br />

• family group practices<br />

• rural outpatient centres<br />

• family medic<strong>in</strong>e centres<br />

• polycl<strong>in</strong>ics.<br />

In rural areas, the feldsher-obstetrical posts are the most<br />

important po<strong>in</strong>ts <strong>of</strong> entry to <strong>health</strong> care, <strong>in</strong>clud<strong>in</strong>g NCD<br />

prevention. In cities, <strong>health</strong> providers <strong>in</strong>clude general and<br />

specialized polycl<strong>in</strong>ics, as well as city hospitals and maternity<br />

homes.<br />
