Broad use is made <strong>of</strong> “recommendations” and “guidel<strong>in</strong>es”.<br />

For example, expert committees have made recommendations<br />

regard<strong>in</strong>g <strong>health</strong>y food, high blood pressure and<br />

cholesterol, dietary guidel<strong>in</strong>es, and goals for mass cater<strong>in</strong>g <strong>in</strong><br />

relation to the fat, salt and sugar content <strong>of</strong> meals provided.<br />

HIA is seen as a vital tool for avoid<strong>in</strong>g negative <strong>in</strong>fluences<br />

on <strong>health</strong> and pick<strong>in</strong>g up opportunities for <strong>health</strong> promotion.<br />

MSAH and the Prime M<strong>in</strong>ister’s <strong>of</strong>fice are to produce<br />

guidel<strong>in</strong>es for HIA <strong>of</strong> central government decisions and<br />

compile models for HIA at municipal level and <strong>in</strong> bus<strong>in</strong>ess<br />

and <strong>in</strong>dustry. In F<strong>in</strong>land, environmental impact assessment<br />

has been mandatory s<strong>in</strong>ce 1994 and should <strong>in</strong>clude the<br />

impact <strong>of</strong> ”<strong>health</strong> hazards”. At the same time, social impact<br />

assessment has been carried out. In the F<strong>in</strong>nish language,<br />

this has a narrower mean<strong>in</strong>g than “social” <strong>in</strong> other countries,<br />

referr<strong>in</strong>g to social protection and social services, and<br />

has been carried out ma<strong>in</strong>ly by Gallup-type report<strong>in</strong>g on,<br />

for example, whether people were will<strong>in</strong>g to pay more for<br />

services. It is suggested (21) that HIA might possibly be carried<br />

out as part <strong>of</strong> a broader “human impact assessment”<br />

and <strong>in</strong> 1999, MSAH published a handbook on the assessment<br />

<strong>of</strong> social and <strong>health</strong> impacts.<br />

2.1.6. Structures and processes for implementation<br />

The previous method <strong>of</strong> top-down guidance through earmarked<br />

resources made the Government, <strong>in</strong> the view <strong>of</strong> a<br />

bureaucrat <strong>of</strong> the time, “almost able to dictate” to the municipalities.<br />

This system was abandoned <strong>in</strong> the early 1990s,<br />

when the municipalities demanded more freedom. Under<br />

the present system, the centre sets overall <strong>policy</strong> direction<br />

and provides expert support for local-level <strong>policy</strong> <strong>development</strong><br />

and tra<strong>in</strong><strong>in</strong>g. There is considerable doubt as to its<br />

success, however, ow<strong>in</strong>g to ensu<strong>in</strong>g <strong>in</strong>equalities, and <strong>in</strong> some<br />

cases a serious lower<strong>in</strong>g <strong>of</strong> standards, <strong>in</strong> services provided.<br />

MSAH is to coord<strong>in</strong> ate and monitor the implementation <strong>of</strong><br />

Health 2015 at national level. In addition, the multisectoral<br />

Advisory Board for Public Health, set up <strong>in</strong> 1997, has two<br />

subgroups, one to support the municipal level and one to<br />

deal with <strong>in</strong>tersectoral action at national level. The permanent<br />

stand<strong>in</strong>g committees <strong>of</strong> MSAH deal<strong>in</strong>g with issues<br />

such as alcohol, drugs and chemical safety <strong>in</strong>clude important<br />

NGOs and are the locus <strong>of</strong> <strong>in</strong>tersectoral action.<br />

STAKES and five smaller agencies were founded <strong>in</strong> 1992,<br />

follow<strong>in</strong>g the dismantl<strong>in</strong>g <strong>of</strong> the previous National Boards<br />

for Health and Welfare. STAKES’ orig<strong>in</strong>al mandate did<br />

not <strong>in</strong>clude <strong>health</strong> promotion, but over the years such<br />

tasks have gradually <strong>in</strong>creased. With the reorganization<br />

<strong>of</strong> the state alcohol monopoly (ALKO) <strong>in</strong> 1996, functions<br />

related to sociological alcohol studies were transferred to<br />

STAKES. When the F<strong>in</strong>nish Centre for Health Promotion<br />

was strengthened and given the task <strong>of</strong> support<strong>in</strong>g citizens,<br />

STAKES was asked to support municipalities <strong>in</strong> develop<strong>in</strong>g<br />

<strong>health</strong> promotion strategies. Support to municipalities takes<br />

the form <strong>of</strong> help<strong>in</strong>g prepare decisions for the allocation <strong>of</strong><br />

<strong>health</strong> promotion fund<strong>in</strong>g; assistance <strong>in</strong> formulat<strong>in</strong>g policies;<br />

develop<strong>in</strong>g <strong>in</strong>dicators and report<strong>in</strong>g; and tra<strong>in</strong><strong>in</strong>g for<br />

<strong>in</strong>teractive ways <strong>of</strong> work<strong>in</strong>g. One <strong>of</strong> STAKES’ first <strong>health</strong><br />

promotion activities relevant to NCD was contribut<strong>in</strong>g to<br />

the <strong>development</strong> <strong>of</strong> social impact assessment.<br />

STAKES assists MSAH <strong>in</strong> <strong>health</strong> <strong>policy</strong> analyses, which <strong>in</strong><br />

1996–1997 <strong>in</strong>cluded a review <strong>of</strong> <strong>in</strong>tersectoral policies <strong>in</strong> F<strong>in</strong>land.<br />

Although it is widely accepted that STAKES has carried<br />

out valuable <strong>policy</strong>-related research, there is less consensus<br />

on how far it has been able to carry out its role <strong>of</strong> support<strong>in</strong>g<br />

the local level. The role and function <strong>of</strong> the <strong>in</strong>stitution is<br />

therefore presently be<strong>in</strong>g re-exam<strong>in</strong>ed.<br />

All urban and rural municipalities <strong>in</strong> F<strong>in</strong>land are members<br />

<strong>of</strong> the Association <strong>of</strong> F<strong>in</strong>nish Local and Regional Authorities,<br />

which also provides services to jo<strong>in</strong>t authorities. The Association<br />

not only promotes the <strong>in</strong>terests <strong>of</strong> local authorities<br />

and seeks to <strong>in</strong>fluence related legislation, both at the<br />

national level and <strong>in</strong> the EU through its Brussels <strong>of</strong>fice, but<br />

provides <strong>in</strong>formation, guidance and tra<strong>in</strong><strong>in</strong>g and facilitates<br />

opportunities for discussion through its networks.<br />
