added weight <strong>of</strong> be<strong>in</strong>g part <strong>of</strong> an <strong>in</strong>ternationally recognized<br />

problem.<br />

A small number <strong>of</strong> highly respected leaders <strong>in</strong> public <strong>health</strong>,<br />

some fulfill<strong>in</strong>g multiple roles, have played an enormously<br />

<strong>in</strong>fluential part <strong>in</strong> guid<strong>in</strong>g <strong>policy</strong> <strong>development</strong>s with<strong>in</strong> the<br />

country and <strong>in</strong> mak<strong>in</strong>g Lithuania’s presence felt at the <strong>in</strong>ternational<br />

level. The role <strong>of</strong> these key movers <strong>in</strong> this relatively<br />

small country has been decisive.<br />

The National Board <strong>of</strong> Health reta<strong>in</strong>s considerable <strong>in</strong>dependence<br />

from the M<strong>in</strong>istry <strong>of</strong> Health and, given its regular<br />

meet<strong>in</strong>gs, would appear to be extremely active. The<br />

strongly expert-led <strong>policy</strong> process facilitates rather efficient<br />

<strong>policy</strong>-mak<strong>in</strong>g but leaves much to be desired regard<strong>in</strong>g the<br />

<strong>in</strong>clusion <strong>of</strong> a wider op<strong>in</strong>ion base.<br />

A valuable and accessible <strong>in</strong>formation base has already been<br />

created, and strong efforts are made to reflect socioeconomic<br />

<strong>in</strong>equalities <strong>in</strong> <strong>health</strong>. More specifically:<br />

• regular surveys <strong>of</strong> <strong>health</strong> behaviour and <strong>health</strong> status<br />

have become standard practice, allow<strong>in</strong>g the monitor<strong>in</strong>g<br />

<strong>of</strong> long-term trends; and<br />

• public <strong>health</strong> reports are published regularly, submitted<br />

to Parliament and discussed openly.<br />

Parliament has played an unusually strong role <strong>in</strong> the area <strong>of</strong><br />

public <strong>health</strong>, <strong>in</strong>clud<strong>in</strong>g the tackl<strong>in</strong>g <strong>of</strong> NCD. The degree <strong>of</strong><br />

party political consensus on <strong>health</strong> issues and confidence <strong>in</strong><br />

national experts means that Parliament is proactive <strong>in</strong> call<strong>in</strong>g<br />

on its national experts to exam<strong>in</strong>e emerg<strong>in</strong>g <strong>health</strong> challenges<br />

and propose solutions. This allows for considerable<br />

conservation <strong>of</strong> energy and effort. Rather than re<strong>in</strong>vent<strong>in</strong>g<br />

the wheel with every change <strong>of</strong> government, <strong>policy</strong>-makers<br />

and planners have built on much <strong>of</strong> the scientific work<br />

previously carried out.<br />

The <strong>in</strong>volvement <strong>of</strong> m<strong>in</strong>isters from other sectors <strong>in</strong> the<br />

<strong>health</strong> debate has been ongo<strong>in</strong>g, enhanc<strong>in</strong>g understand<strong>in</strong>g<br />

<strong>of</strong> the need for <strong>in</strong>tersectoral action. It is not clear how far<br />

this translates <strong>in</strong>to specific action, or if awareness <strong>of</strong> the<br />

need for cross sector collaboration trickles down with<strong>in</strong> the<br />

m<strong>in</strong>istries or to the agencies for which they are responsible.<br />

Although the sett<strong>in</strong>gs approach seems to have been used<br />

quite widely <strong>in</strong> schools and to some extent at the city<br />

level, relatively little focus seems to have been given to the<br />

workplace. Given the potential <strong>of</strong> the workplace as a sett<strong>in</strong>g<br />

for tackl<strong>in</strong>g NCD, and coupled with the fact that Lithuania is<br />

undergo<strong>in</strong>g rapid economic growth, develop<strong>in</strong>g the concept<br />

<strong>of</strong> “socially responsible corporations” might <strong>of</strong>fer considerable<br />

benefits.<br />

The medical and nurs<strong>in</strong>g pr<strong>of</strong>essions have cont<strong>in</strong>ued to<br />

provide basic care under difficult conditions. Attempts are<br />

be<strong>in</strong>g made to enhance their capacity for <strong>health</strong> promotion.<br />

However, if top priority is not given to strengthen<strong>in</strong>g PHC,<br />

it is difficult to see how the NCD <strong>policy</strong> can be effectively<br />

implemented.<br />

The sett<strong>in</strong>g <strong>of</strong> clear objectives and quantified targets related<br />

to NCD has focused the <strong>policy</strong> discussion and encouraged<br />

adherence to strict epidemiological monitor<strong>in</strong>g and evaluation.<br />

More attention will need to be given to evaluat<strong>in</strong>g the<br />

process <strong>of</strong> NCD <strong>policy</strong> <strong>development</strong> <strong>in</strong> terms <strong>of</strong> whether<br />

or not the necessary human and f<strong>in</strong>ancial resources were <strong>in</strong><br />

place, whether the various partners had a clear understand<strong>in</strong>g<br />

<strong>of</strong> their expected roles, and what has worked or not<br />

worked and why.<br />

Regular <strong>health</strong> <strong>policy</strong> conferences broaden the debate, as<br />

do the mass media. More could be done to popularize<br />

<strong>in</strong>formation on NCD prevention and <strong>health</strong> promotion and<br />

<strong>in</strong>volve the public <strong>in</strong> def<strong>in</strong><strong>in</strong>g the challenges and consider<strong>in</strong>g<br />

the <strong>policy</strong> options.<br />
