• Exam<strong>in</strong><strong>in</strong>g the costs. The economic argumentation for<br />
tackl<strong>in</strong>g NCD has not been made sufficiently strongly.<br />
More research is needed <strong>in</strong>to the economic implications<br />
<strong>of</strong> NCD and the cost-effectiveness <strong>of</strong> NCD <strong>in</strong>terventions.<br />
• Improv<strong>in</strong>g the l<strong>in</strong>ks between <strong>policy</strong>-mak<strong>in</strong>g and research.<br />
Close <strong>in</strong>teraction between researchers and <strong>policy</strong>makers<br />
can be decisive <strong>in</strong> the use <strong>of</strong> scientific evidence<br />
for NCD <strong>policy</strong>-mak<strong>in</strong>g. Channels to enhance such<br />
<strong>in</strong>teraction could encourage the production and use <strong>of</strong><br />
<strong>policy</strong>-relevant research.<br />
• Learn<strong>in</strong>g from peers. Peer review has proved effective <strong>in</strong><br />
F<strong>in</strong>land and Ireland and could be one way for countries<br />
to assess their exist<strong>in</strong>g NCD policies and the potential<br />
for improvement.<br />
Adopt<strong>in</strong>g the most effective approach<br />
The <strong>European</strong> NCD Strategy states that “It is possible for<br />
all countries across the Region with very different <strong>in</strong>comes<br />
and capacities to effectively combat NCD”. The most effective<br />
approach should be adopted <strong>in</strong> a particular situation.<br />
This may require maximization <strong>of</strong> opportunity rather than<br />
<strong>in</strong>creased resources.<br />
• Mak<strong>in</strong>g the best <strong>of</strong> exist<strong>in</strong>g assets. An assessment not<br />
only <strong>of</strong> high-priority needs but also <strong>of</strong> available assets<br />
could <strong>in</strong>dicate where effective <strong>in</strong>terventions could be<br />
made.<br />
• Borrow<strong>in</strong>g and jo<strong>in</strong><strong>in</strong>g. This should <strong>in</strong>clude a quick review<br />
<strong>of</strong> other ongo<strong>in</strong>g policies (and not only <strong>in</strong> the <strong>health</strong><br />
field) on which NCD <strong>policy</strong> could “piggy-back” or <strong>in</strong>to<br />
which an NCD <strong>policy</strong> component could be added.<br />
• Look<strong>in</strong>g for quick w<strong>in</strong>s. An <strong>in</strong>itial focus on selected quick<br />
w<strong>in</strong>s would encourage the long-term NCD <strong>policy</strong> process.<br />
Be<strong>in</strong>g aware <strong>of</strong> the wider <strong>policy</strong> environment<br />
Increas<strong>in</strong>gly, decision-mak<strong>in</strong>g <strong>in</strong> the <strong>health</strong> sector is <strong>in</strong>fluenced<br />
by decisions <strong>in</strong> the broader national <strong>policy</strong> environment<br />
or at <strong>in</strong>ternational level. Some <strong>of</strong> the future challenges<br />
and opportunities were exam<strong>in</strong>ed <strong>in</strong> the <strong>in</strong>troduction to<br />
this chapter. Unless changes <strong>in</strong> the <strong>policy</strong> environment are<br />
adequately assessed, NCD <strong>policy</strong> could be derailed.<br />
• Macro changes <strong>in</strong> areas such a public adm<strong>in</strong>istration,<br />
management, and education need to be assessed for<br />
their possible impact on NCD <strong>policy</strong>-mak<strong>in</strong>g.<br />
• Work<strong>in</strong>g with the private sector <strong>of</strong>fers considerable<br />
potential for tackl<strong>in</strong>g NCD, but should be guided by a<br />
clear code <strong>of</strong> ethical behaviour.<br />
• In low- and medium-<strong>in</strong>come countries, new forms <strong>of</strong><br />
partnership need to be developed between <strong>in</strong>ternational<br />
organizations, bilateral donors and national stakeholders<br />
<strong>in</strong> NCD <strong>policy</strong>.<br />
• The potential impact <strong>of</strong> globalization on NCD needs to<br />
be more closely monitored and evaluated.<br />
• New communication technologies <strong>of</strong>fer a huge potential<br />
for access to <strong>in</strong>formation, consultation and network<strong>in</strong>g,<br />
but also hold considerable threats. The opportunities<br />
and challenges for NCD <strong>of</strong> this <strong>in</strong>formation explosion<br />
need to be monitored and exploited as appropriate.<br />
• NCD <strong>policy</strong>-makers could advocate for more openness<br />
<strong>in</strong> the EU for l<strong>in</strong>k<strong>in</strong>g NCD <strong>policy</strong> to <strong>development</strong>al issues<br />
through the structural and other funds.<br />
It can and must be done<br />
Most countries are fac<strong>in</strong>g what may appear to be overwhelm<strong>in</strong>g<br />
challenges <strong>of</strong> f<strong>in</strong>ancial str<strong>in</strong>gency, loom<strong>in</strong>g unemployment<br />
and the disaffection <strong>of</strong> certa<strong>in</strong> social groups, and a<br />
climate <strong>in</strong> which many <strong>of</strong> the str<strong>in</strong>gs appear to be not <strong>in</strong> the<br />
hands <strong>of</strong> national authorities but <strong>in</strong> those <strong>of</strong> supranational<br />
and <strong>in</strong>ternational organizations and corporations. In this<br />
difficult situation, tackl<strong>in</strong>g NCD could be considered not to<br />
be a priority or to require too much effort and resources<br />
when urgent economic issues must be dealt with.<br />
