Clinical Directorates I GEMS<br />

<strong>St</strong>. James’s continues to submit data for inclusion in the<br />

association of Coloproctology of Great Britain and Ireland’s<br />

bowel cancer audit. Data from 2009/<strong>2010</strong> will be included in<br />

their next report due to be published in 2011.<br />

The availability of expertise in colorectal stenting in the<br />

emergency setting has allowed patients with large bowel<br />

obstruction to avoid a colostomy using stenting as a bridge<br />

to surgery.<br />

The colorectal cancer nurses Delia Flannery and Katrina<br />

O’Connor (Acting) continue to provide patient focused care<br />

from diagnosis, through treatment and onwards to nurse<br />

led surveillance. The follow up clinic workload continued to<br />

increase in <strong>2010</strong>; 252 patients were seen in the clinic which<br />

is almost a 25% increase since 2009. A patient satisfaction<br />

survey report was completed in <strong>2010</strong> to assess patient’s<br />

opinion of the nurse led clinic.<br />

The stoma care department has two full time nurse specialists<br />

positions; AnneMarie <strong>St</strong>uart, Siobhan Mc Govern (·5)<br />

and Anna Fearon (∙5) who provide pre and post operative<br />

counselling, care and education to all patients who may<br />

potentially require stoma formation. The department received<br />

235 new referrals in <strong>2010</strong>. The number of patients seen in<br />

their daily nurse led OPD clinics was 509.<br />

Renal Dialysis Unit<br />

Since Oct 2008 <strong>St</strong>. James’s <strong>Hospital</strong> now provide patients<br />

with an improved treatment called Online Haemodiafi ltration.<br />

The benefi ts of Online Haemodiafi ltration versus standard<br />

haemodialysis are:<br />

• Online HDF stabilises blood pressure using the process pre<br />

dilution. This creates better cardiac stability- therefore suited<br />

to the unstable hypotensive, hypervolaemic/pulmonary<br />

oedema patient, or patient in ICU<br />

<strong>St</strong>oma Nursing Department<br />

The <strong>St</strong>oma nursing Department in <strong>St</strong>. James’s <strong>Hospital</strong><br />

provides a responsive, supportive and comprehensive nursing<br />

service to patients who have existing stomas or who require<br />

stoma formation, or reconstructive bowel and bladder surgery<br />

and management of enterocutaneous fi stulae.<br />

• Online HDF allows greater blood clearances of Urea<br />

and Creatinine due to convective transport in HDF in<br />

comparison to diffusion transport in haemodialysis<br />
