Payment deductions will be calculated by transferring in<strong>for</strong>mation on all unsatisfactory workrequirements from the EVALUATION WORK SHEETs to a MONTHLY PAYMENT DEDUCTIONFORM. A sample MONTHLY PAYMENT DEDUCTION FORM is attached.b. ODRs will be calculated <strong>for</strong> each work requirement <strong>for</strong> the overall per<strong>for</strong>mance of scheduledstored products pest control services by summarizing the inspection results from all EVALUATIONWORK SHEETs and using the following <strong>for</strong>mula:ODR = Total Number of Defects Observed in Sample x 100Number of Services Sampled(1) If the ODR <strong>for</strong> a work requirement is less than or equal to its MADR, the Contractor'soverall per<strong>for</strong>mance of that requirement is satisfactory. If the ODR is less than ½ the MADR, the QAEshould recommend to the COR to notify the Contractor that per<strong>for</strong>mance is excellent and to keep up thegood work. Consider using reduced surveillance <strong>for</strong> the coming evaluation period.(2) If the ODR <strong>for</strong> a work requirement is greater than its MADR, overall per<strong>for</strong>mance of thatrequirement is unsatisfactory and the QAE should recommend to the COR that a CDR be issued to theContractor, or that stronger action be taken. NOTE: Since the MADR <strong>for</strong> quality of work is zero, anoverall unsatisfactory quality grade on any given treatment will result in an overall unsatisfactory qualityrating <strong>for</strong> the entire contract requirement. Increased surveillance should be used <strong>for</strong> the coming evaluationperiod (see paragraph 5 above).QA-31
