General. In most all instances where the quality of work is considered unsatisfactory, timelyservice will also be considered unsatisfactory. <strong>Pest</strong> management records will be inspected <strong>for</strong> eachsampled service at the end of the month when the invoice is received, and spot checks will be made duringthe month as required to verify that these records are routinely being completed within 24 hours of theservice being completed.d. Evaluation Work Sheets. Grades of either satisfactory (S) or unsatisfactory (U) will be recordedon the attached EVALUATION WORK SHEET <strong>for</strong> each work requirement. To avoid confusion, aseparate work sheet will be used <strong>for</strong> each type of treatment. A brief but complete description of anynoted defects, including the quantity of unsatisfactory work, will be provided and retreatment in<strong>for</strong>mationrecorded, if appropriate. Any violations of regulations or contract standards, such as pesticides not beingapplied per label instructions, will be annotated in the remarks column. Provide copies of all negativeEVALUATION WORK SHEETs to the Contractor.e. Unscheduled Inspections. Unscheduled inspections may be conducted on any service location,but should be limited to those where control problems would normally be expected or have been notedpreviously. Unscheduled inspections should be documented on a separate EVALUATION WORKSHEET from that used <strong>for</strong> planned sampling.f. Validated Customer Complaints. The QAE will validate each customer complaint received on thestandard customer complaint <strong>for</strong>m. Normally, site visits will be required to validate complaints.g. Call Backs. Retreatments will almost always be required and must be completed by theContractor within 24 hours of notification unless delays were requested by building occupants. Each siteretreated will be inspected by the QAE to ensure the proper level of control has been achieved andappropriate notations completed on the EVALUATION WORK SHEET.QA-66
