NAVFAC MO-327 and theService RSED (V3.2) implementation guide provide guidance in determining the Contractor's overallmonthly per<strong>for</strong>mance <strong>for</strong> each service; how to use the PRS table and the QAE's inspection results tocalculate the total payment due <strong>for</strong> each service; and how to go about correcting problem areas ofper<strong>for</strong>mance. This paragraph provides additional in<strong>for</strong>mation on the completion of the MONTHLYPAYMENT DEDUCTION FORMs included in the sample QA plans, and includes a sample monthlysummary report. (need Component equivalent document citation)A. Monthly Payment Deduction Form. These <strong>for</strong>ms are very useful <strong>for</strong> summarizing the results ofeach month's inspections and illustrate how the CONSEQUENCES OF CONTRACTOR'S FAILURETO PERFORM REQUIRED SERVICES and ESTIMATING THE PRICE OF NONPERFORMEDOR UNSATISFACTORY WORK clauses, the Schedule of Deductions, the PRS table, and the QAE'scompleted EVALUATION WORK SHEETs are all used in calculating the total payment due <strong>for</strong> eachcontract requirement. The <strong>for</strong>mat of these <strong>for</strong>ms should be tailored by the user. Other sample <strong>for</strong>matsmay be found in the NAVFAC MO-327, the NAVFAC RSED implementation guide, and as mentionedpreviously, computer programs are available which will per<strong>for</strong>m and document basically the samecalculations.B. Analysis of Results. The end result of the monthly inspection process is the overall evaluation ofthe Contractor's per<strong>for</strong>mance <strong>for</strong> each pest control service inspected. Such an evaluation provides asummary of the Contractor's per<strong>for</strong>mance to the contracting officer, COR, QAE, customerrepresentatives, and the Contractor. Overall per<strong>for</strong>mance is important in determining whether to increase,decrease, or maintain surveillance at the same level; whether to issue one or more CDRs to the Contractoror take stronger administrative actions; and points out service areas which require greater QAE andContractor QC emphasis during the next evaluation period. There<strong>for</strong>e, at the end of each month the QAEshould complete and <strong>for</strong>ward <strong>for</strong> the COR's approval a MONTHLY SUMMARY REPORT, in a <strong>for</strong>matsimilar to the sample shown in Table 1. Almost all the in<strong>for</strong>mation required to complete this summary canbe taken directly from the MONTHLY PAYMENT DEDUCTION FORMs included with the sampleQA plans.QA-75
