Prior to the beginning of the month, the Contractor's approved schedule will beused to determine the adult mosquito surveillances scheduled to be accomplished, and the appropriatenumber of samples based on the level of surveillance being used. Locations will be chosen on a rotatingbasis so that selection will be consistent from month to month and all surveillance locations will periodicallybe included in the sample.8. Evaluation Procedures. The QAE will inspect each selected trap location by 9:00 a.m., or as soonthereafter as possible, to verify they are operating properly and have been emptied. Appropriatenotations will be made on the attached EVALUATION WORK SHEET. At noon, the QAE willdetermine if written tallies have been provided <strong>for</strong> each sampled trap and appropriate in<strong>for</strong>mation hasbeen provided. Work quality will be graded unsatisfactory overall if the inspected trap is inoperative ornot emptied, or if the written tally is not complete or submitted on time. <strong>Pest</strong> management records <strong>for</strong>each sampled trap will be inspected at the end of the month when the invoice is received; spot checks willbe made during the month as required to verify records are routinely being completed within 24 hours ofper<strong>for</strong>mance.a. Evaluation Work Sheets. Trap and tally in<strong>for</strong>mation, and grades of either satisfactory (S) orunsatisfactory (U), will be recorded on the attached EVALUATION WORK SHEET <strong>for</strong> both workrequirements. A brief but complete description of any noted defects will be provided. Copies of allnegative EVALUATION WORK SHEETs will be furnished to the Contractor.b. Unscheduled Inspections. Unscheduled inspections may be conducted on any trap location, butshould be limited to those where problems have been previously noted. Unscheduled inspections shouldbe documented on a separate EVALUATION WORK SHEET from that used <strong>for</strong> planned sampling.9. Analysis of Results. At the end of the month, the QAE will summarize the results of the month'sinspections and calculate ODRs and recommended payment deductions <strong>for</strong> each work requirement on aMONTHLY PAYMENT DEDUCTION FORM. A sample MONTHLY PAYMENT DEDUCTIONFORM is attached.a. If the ODR <strong>for</strong> a work requirement is less than or equal to its MADR, overall per<strong>for</strong>mance of thatrequirement is satisfactory. If the ODR is less than ½ the MADR, the QAE should recommend to theCOR to notify the Contractor that per<strong>for</strong>mance is excellent and to keep up the good work. Considerusing reduced surveillance the coming evaluation period.b. If the ODR <strong>for</strong> a work requirement is greater than the MADR, overall per<strong>for</strong>mance of thatrequirement is unsatisfactory and the QAE should recommend to the COR that a CDR be issued to theContractor, or that stronger action be taken. Increased surveillance should be used <strong>for</strong> the comingevaluation period (see paragraph 5 above).QA-35
